Abstract: The patient in this case is a 50-year-old female with a perforated right ear tympanic membrane caused by a cold and fever in childhood, and has had recurrent pus flow for decades, with symptoms such as pus flow, ear pain, and hearing loss again due to unprotected ear bathing. He was diagnosed with chronic suppurative otitis media in the right ear and was cured after 10 days of medication such as levofloxacin, cefixime and physical therapy. In our daily life, we should pay attention to maintain good habits to protect the ear and prevent otitis media. The patient’s right ear drum was perforated due to a cold and fever as a child, and she had repeatedly had pus in her right ear for decades. 10 days ago, she did not protect her right ear in the shower, which led to the recurrence of pus flow, yellow pus, no odor, occasional ear pain, hearing loss, no tinnitus, no dizziness, nausea, vomiting, no fever, cough, etc. On examination, we saw yellow pus in the ear canal, and the tympanic membrane was not visible. We gave hydrogen peroxide to flush out the pus from the ear canal, and we could see a large perforation in the center of the tympanic membrane, some remnants of the surrounding tympanic membrane, congestion in the middle ear and tympanic membrane, and congestion in the ear canal, and diagnosed chronic suppurative otitis media in the right ear. Second, the treatment goes through Treating the patient with purulent otitis media of the right ear and pus in the ear canal should be taken as a cleaning treatment. Oral cefixime treatment is combined with gas-based physiotherapy treatment in the right ear canal. Gas-based physiotherapy has the therapeutic effect of sterilizing and anti-inflammatory and drying the affected ear. Patients should cooperate fully during the treatment period, keep the right ear completely dry and clean, and avoid water in the ear canal. III. Treatment effect There will be pus in the ear canal of chronic suppurative otitis media relapse, which is produced by the inflammation in the middle ear of the patient and flows out from the perforated eardrum. After treatment, the flow of pus from the affected ear stops, the inflammation in the middle ear is controlled, and the external ear canal and middle ear can be seen to be in a dry state. The central tympanic membrane perforation was clearly observed through endoscopic examination, and part of the remaining tympanic membrane was visible. The middle ear and the remaining tympanic membrane were not congested, and there was no purulent discharge in the middle ear. The patient’s hearing improved and there was no pain in the affected ear. Precautions We are glad that the patient’s hearing has improved and the discomfort has been relieved after treatment, but chronic suppurative otitis media is mainly a chronic old middle ear lesion, and the patient still needs to pay attention in daily life: 1. It is generally recommended that patients with chronic suppurative otitis media wear ear plugs with a tighter pressure, or ear muffs with better protection, and keep the ear canal dry to avoid water entering the ear canal and middle ear.
