The treatment of plasmacytoid mastitis by the “clearing and eliminating method” in Chinese medicine

Chinese herbal medicine is preferred for the treatment of plasmacytoid mastitis (acne canker sores), which has good efficacy. In particular, the application of the “clearing and eliminating method” in the treatment of acanthotic canker sores in clinical practice has achieved good results and reflected the characteristics of TCM in the treatment of breast diseases, breaking the traditional method of warming Yang, dissolving phlegm and dispersing nodules – using Yang He Tang as the basic formula for the treatment of this disease, and providing a new way of thinking for the treatment of this disease in TCM. The treatment of this disease is based on the Yang He Tang formula, which provides a new way of thinking for the treatment of this disease. In terms of etiology and pathogenesis, he inherited the understanding of Professor Gu Bohua on the etiology and pathogenesis of this disease, that is, he believed that the nipple is sunken and deformed due to emotional and mental discomfort, liver qi stagnation, blood stagnation and blood stasis, coalescing into lumps, which turn into heat and steam to form abscesses and fistulas after ulceration, and also due to qi stagnation turning into fire and forcing blood to move delusionally, resulting in nipple blood overflow. In terms of treatment, it is important to learn from the “Compendium of Ulcerology” that “the treatment of carbuncles must be eliminated if they are not yet formed, and if they are treated early, they can be eliminated invisibly”. Therefore, in Chinese medicine surgery, “elimination”, “support” and “supplementation” are the general rules of treatment, and in the long-term development of Chinese medicine surgery theory and practice, we always insist on the important principle of “elimination is the most important”. In the long-term development of the theory and practice of Chinese medicine surgery, it has always insisted on the important treatment principle of “elimination is the most important”. The elimination method can not only make the external ulcers disappear internally, but also play a role in dissipating the stasis and phlegm coagulation that accompany sores and carbuncles. If we mechanically apply the elimination, support and supplementation methods to the early, middle and late stages of sores, we are bound to fall into a rigid diagnostic and therapeutic pattern, which is contrary to the principles of diagnosis and treatment in Chinese medicine and hinders the development of clinical thinking skills and affects the clinical efficacy. The elimination method should be combined with the use of the TO method and the tonic method according to changes in the condition. In the early, middle and late stages of sores and ulcers, according to the condition, either the elimination method should be combined with the TO method, or the elimination method should be used together with the TO method, or the elimination method should be used together with the TO method, and applied flexibly according to the changes of the main symptoms. The application of the “Qing and elimination method” in the treatment of this disease is a manifestation of this treatment rule. “In the early stage of application, it is important to be early; in the pus-forming stage, it is important to eliminate; in the fistula stage, it is important to dissolve phlegm and disperse nodules to eliminate stiffness. The elimination method can be applied not only in the early stage when acne canker is not pus-forming, but also in the abscess stage and fistula stage, which can achieve twice the effect with half the effort. The abscess stage of acne vulgaris is characterized by softening of the breast lump, formation of an abscess, and the rupture of the pus, which is often interspersed with acne-like or lipid-like material. The abscess can be one or several, mostly under the areola, and in deeper cases it can spread to the posterior space of the breast, where it successively becomes septic and breaks down and can heal temporarily, often repeatedly. For this period, the use of elimination method is different from the initial period, still the main method of elimination, accompanied by a small amount of agent, generally the amount of agent should be small, the amount of elimination agent should be large, agent and elimination agent together, in order to achieve the purpose of pus penetration swelling elimination. The treatment is based on the principles of draining the liver, resolving turbidity, relieving dampness and dipping fire. It has good efficacy in clinical application. If there is much pus and fluctuating sensation, it should be first cut and drained under local anesthesia, so that the thick out is unobstructed and the patient feels slightly comfortable, and it is important not to make a large area of cut rot in this period, which will cause unsightly after-effects to the patient’s breast. After the operation, the drainage is done with eighty-two Dan medicine line, red oil paste gauze cover paste. In conclusion, the treatment of plasmacytoid mastitis should give full play to the advantages of TCM surgical treatment, combining the identification of disease and evidence, flexible combination of internal and external treatment methods, good at grasping the main contradictions in the development of the disease, the rational use of the three major treatment principles of “elimination, support, tonic”, in order to further improve the clinical efficacy of TCM treatment of this disease This is to further improve the clinical efficacy of TCM treatment.
