Postpartum pubic symphysis separation should be considered. Postpartum pubic separation is a common complication of pregnancy, usually occurring in patients with difficult or emergency deliveries. After delivery, patients may have localised pain in the anterior pelvic region, swelling, restricted movement, radiating low back, inguinal region pain, positive pelvic squeeze separation test due to the rupture of the pubic symphysis, and may also present with pain in the pubic arch region, instability of the pelvic ring on movement, and audible bone rubbing sounds.
In addition, the pelvis is unable to bear weight due to impaired pelvic stability, which may result in weakness and a wobbly gait. The clinical presentation combined with relevant imaging studies such as x-ray, ultrasound, pelvic CT and pelvic MRI can confirm the diagnosis.
