Complex portal hypertension 6 (portal thrombosis after inferior vena cava filter, stent placement)

  Case description: Male, 36 years old, hospitalization number 4415960. Complaint: Intermittent vomiting of blood and black stool for more than 9 months.  Past and present history: He underwent inferior vena cava filter and stent placement in 2004 and 2009 respectively for lower limb deep vein thrombosis, and the swelling of the lower limbs improved after the surgery. On admission: severe anemia, massive ascites, splenomegaly, hematocrit 3.6 g. Vital signs were stable. After 15 days of preoperative preparation, a giant splenectomy, selective combined dissection and liver biopsy were performed under general anesthesia on 2012-02-16. Intraoperatively, he saw 2000 ml of ascites, normal liver, giant spleen, extensive dilatation of esophagogastric fundic vein penetration and splenic vein with thrombosis, 1 cm diameter of left gastric vein, open umbilical vein, FPP 48 cmH2O, normal blood flow of intra-abdominal organs, and no infarction of spleen. He was discharged from the hospital on 2012-03-04 after a smooth postoperative recovery.  Follow-up: No discomfort until 2012-11-16, with normal blood count and biochemical set.  Follow-up: He was referred on 03-26-2013 with multiple black stools after binge drinking, no blood vomiting and severe anemia. At admission, vital signs were stable, no ascites, mild swelling of the left lower limbs, and normal dorsalis pedis artery pulsation of both lower limbs. He was discharged after one week of conservative treatment with yellow stools.  Follow-up: No discomfort until November 29, 2013, and normal blood party rules and biochemistry.
