Wake up beautiful women suddenly can not close their eyes? Who caused this trouble?

A few days ago, a beautiful girl came to the clinic and said anxiously that she suddenly couldn’t close her right eye after waking up, and at first she thought it would improve after a while because she hadn’t rested well, but she didn’t expect it to recover after a day. An examination: Bell’s sign was positive (i.e., when the eye is closed with force, the eyeball turns upward to reveal the white sclera), the right cheek does not move well, the mouth is crooked, the eye fissure is enlarged, the nasolabial fissure is flattened, the sick side cannot frown, close the eyes, puff and pout, and the tongue is not centered. The medical history of this beautiful woman revealed that she had caught a cold in recent days. Based on clinical experience, it was not difficult to diagnose facial nerve palsy. Although facial nerve palsy belongs to the category of neurological diseases, ophthalmology should not ignore this disease, and there are two types of clinical nerve palsy: central and peripheral. Central facial palsy is caused by cerebrovascular disease, in which facial palsy is a typical symptom of the disease, accompanied by dizziness, numbness of the limbs, and slurred speech. In contrast, peripheral facial palsy is simple facial palsy with no other symptoms. There are many causes of peripheral facial palsy, such as cold, exposure to cold, viral infection, and diabetes mellitus. The incidence of peripheral facial palsy is high in clinical practice. The pretty girl broke the casserole and asked me, “Doctor, why does facial palsy occur when you can’t close your eyes?” I answered seriously, “Because there is a muscle on the eyelid that is responsible for closing the eyes, and this muscle is called the orbicularis oculi muscle, which is innervated by the facial nerve. When you don’t keep warm or cool, you will catch a cold, and then the virus will invade the facial nerve, which will impair the function of the facial nerve and prevent it from innervating the orbicularis oris muscle, thus affecting eye closure. In terms of treatment, for central facial palsy, treatment will be targeted according to cerebrovascular disease based on brain examination results. For peripheral facial palsy, depending on the cause, anti-viral drugs are used for viral infection, along with drugs that promote the recovery of nerve function, such as vitamin B1 and B12, methoxamine or adenosine estimation amine. In diabetic patients, the key is to lower blood sugar. In addition, acupuncture, physical therapy and massage are also effective as rehabilitation treatments. It should be emphasized that attention must be paid to the occurrence of exposure keratitis, because the eyelids are not fully closed or cannot be closed, transient movements and corneal reflexes disappear, the cornea is exposed for a long time, which can easily lead to intraocular infection and damage to the cornea, so eye protection is very important, reduce eye use, wear sunglasses for protection when you go out, as well as drops of eye drops with lubricating, anti-inflammatory and nutritional effects, and erythromycin eye ointment, eye shields or gauze to protect the cornea when sleeping. or cover with gauze to protect the cornea. Finally, I also instructed her to keep warm and rest during the treatment period, not to stay up late, and to avoid spicy and light diet. After 3 days of treatment, the patient’s symptoms such as incomplete eyelid closure were significantly relieved, so this beautiful woman was instructed to continue treatment for 3 more days.
