Benign breast enlargement caused by hyperplasia of the male breast tissue is called gynecomastia. The clinical presentation is progressive unilateral or bilateral breast enlargement or lumpiness, which may be associated with pain and significant psychological frustration and distress. Gynecomastia does not respond well to diet and exercise, and surgery is often the first line of treatment. Transaxillary route liposuction (via vacuum suction cannula) can result in a good breast contour with only a small axillary scar. Even if the procedure is very successful, there is still visible postoperative scarring with this approach. To avoid visible scarring, liposuction treatment can be performed via the umbilical route. That is, a vertical incision is made at the 12 o’clock position of the umbilicus and the hypertrophic tissue is removed with the classic swollen liposuction technique. After liposuction, a corset is wrapped and compressed for 6 to 8 weeks to help the skin contract. The advantage of the transumbilical route is that one incision can reach both sides of the chest, leaving only an inconspicuous umbilical scar after surgery and no traces of chest surgery.
