I have been wanting to write this article for a long time, but what finally prompted me to write it was the tragic experience of a recent outpatient, a 15-year-old girl from Gansu. She had a post-operative tibial spine avulsion fracture with knee adhesions, and had actually received inhumane treatment at a local rehabilitation center for seven months, crying every day when her leg was broken, crying during the daytime treatment, and continuing to cry when she returned home and thought about being sad. Seven months down weight loss of 25 pounds. The result of the 7 months of treatment was still that her legs could not be straightened or flexed. I accepted this patient and said to the little girl, “We will not let you shed a single tear when you are treated with us. In my clinic, I often see many patients with adhesions and stiffness in the knee and elbow joints, and before they come to our department, they receive many manipulative treatments that cause them severe pain. In the end, these patients are often not treated as well as they should be. Their doctors often told the patients that they would not get better without pain, and the patients believed that they would not be able to pull the adhesions apart without pain, so they endured the pain from the treatment every day. I often tell my patients that this is like a hunchback, can it be flattened by using two door plates? Imagine if an elbow or knee joint that cannot be flexed can be restored by a simple and brutal hard wrenching, then there is no need to go to a hospital rehabilitation department to receive professional treatment for such physical work. Joint adhesions and stiffness are called joint contractures in medical terminology. The causes of joint contractures are complex, with both intra- and extra-articular causes. The treatment strategies and techniques for different causes and stages of joint contracture will vary. These need to be determined by the physician through careful examination and professional analysis. Treatment techniques for joint contractures include soft tissue release, joint release, joint traction, and continuous progressive distraction. Contrary to what many lay people express, none of these treatment techniques for joint contracture cause significant pain to the patient. Not only is it painless, but the joint adhesions can always gain significant mobility in one treatment, which is “no pain, more gain! Pain-free rehabilitation treatment is exactly the “humanized” rehabilitation treatment concept for joint contractures that the Department of Rehabilitation of the Ninth Hospital has been promoting nationwide for years. The Department of Rehabilitation in our hospital was the first to introduce the advanced Nordic school Kaltenborn joint release technique from the University of Southern California, and has summarized a set of rehabilitation treatment strategies and related techniques for joint contractures. Every year, many doctors from all over China come to Shanghai Jiu Hospital to learn this pain-free concept and joint release technique. From March 2009 to December 2013, a total of 52 trainees were trained, covering half of China, with a total of 44 hospitals in 16 provinces, 1 autonomous region and 2 municipalities directly under the central government. This painless rehabilitation treatment concept and technique has benefited patients with joint contractures all over China. Postscript: As of this writing, the little girl has been treated with us for 5 times without shedding a single tear, and has gone from only 15 degrees of flexion when she first arrived to 50 degrees of flexion. The girl’s and the patient’s mother’s faces, which had been tense, finally showed a smile. Through the joint efforts of the doctor and patient, we hope to return the little girl to a sound function and a high quality of life.
