Is a heart stent a “virtuous procedure”?

I have done an introduction and explanation on the choice of stent and bypass in Yangcheng Evening News, but I didn’t expect some people to publish some irresponsible remarks for their own benefit, after all, it is still a disaster of interest! In 2013, there was a rumor that “heart stent is a virtuous surgery”, which was widely circulated, “Heart stents are time bombs in the human body” and “you will die faster with a heart stent” …… Dr. Cui Song said that this rumor has done great harm to the credibility of medicine. The truth is that heart stenting is still widely used in developed countries and the technology is mature and effective. Rumors damage medical credibility Dr. Cui Song, chief physician of cardiology, made a special slide show and video clip with the title “Let’s reacquaint ourselves with cardiac stenting” to explain to the audience why this is a complete rumor. Dr. Cui Song was particularly saddened by the fact that this rumor has done great harm to the credibility of medicine, and even some unknown doctors have forwarded it in WeChat. “Rumors are worse than ignorance, because at the critical moment, it may prevent you from doing the right thing, and it may be related to a person’s life.” From the reason this rumor was generated, it was actually Dr. So-and-so promoting his own invention of bioelectric resonance therapy, which he claimed could cure coronary heart disease. Regardless of the science and effectiveness of the therapy, it is wrong to “scandalize” the heart stent surgery and scare patients with sensational things in order to elevate themselves, and the end result is to make patients take their own treatments without medication or surgery. Why do we need heart stenting? Dr. Cui Song opened the slides and told the audience. Coronary heart disease is the culprit of sudden death” Nowadays, many people die suddenly, including Hou Yuewen, Ma Ji, Gao Xiumin, Gu Yue, and Li Ming, the chairman of Pony Run, who just passed away. Sudden death is sudden death, of which, 90% are sudden cardiac death, and in sudden cardiac death, myocardial infarction is the most people, coronary heart disease is the culprit of sudden death.” The full name of coronary heart disease is coronary atherosclerotic heart disease, is one of the most common heart disease, refers to the myocardial dysfunction or organic lesions caused by the narrowing of coronary arteries, insufficient blood supply, so also known as ischemic heart disease. The occurrence of coronary heart disease is closely related to the degree and location of atherosclerotic stenosis of coronary arteries, as well as hypertension, diabetes and other diseases, as well as excessive obesity and poor lifestyle habits are the main factors triggering the disease. Coronary heart disease is one of the diseases with the highest mortality rate worldwide, and according to a 2011 report by the World Health Organization, China has the second highest number of coronary heart disease deaths in the world. “There is a network of blood vessels in the heart, and if blood sugar, lipids and uric acid are high, the blood is more turbid inside, and slowly these things are deposited, like sediment in a riverbed, and more and more plaques appear, causing the blood vessels to become thinner and thinner.” Dr. Cui Song said, “A blocked vascular network is like a blocked water pipe, causing a regional water stoppage, which can lead to myocardial infarction if the ischemia lasts too long, and myocardial infarction has a very high incidence and lethality.” Rumor has it that some doctors abroad can remove the plaque with a vascular clamp after the blood vessel has “blocked up”, which Dr. Cui Song clearly said is impossible. How to treat? Dr. Cui Song said the first method is medication, followed by interventional and surgical treatment. “If you compare the blood vessel to a sewer, asking workers to pass one is to install a stent, and if you change the heel tube, it is a bridge.” Director Cui Song explained in depth, “Installing a stent is not only not a deficiency surgery, but also a life-saving surgery. Because of this surgery, many people are rescued from the ghost gate.” The procedure for cardiac stenting involves puncturing a blood vessel, causing the catheter to travel forward in the vessel, reaching the opening of the coronary artery, using a special delivery system to deliver the stent to the site where it needs to be placed, placing it, withdrawing the catheter and ending the procedure. The patient undergoes the procedure under local anesthesia and is usually out of bed 24 hours after the puncture. For patients with angina pectoris and myocardial infarction, time is life. In order to increase the success rate of resuscitation in such patients and minimize the risk of death, it is necessary to receive vascular opening as fast as possible. Therefore, all major hospitals have green channels, and once a myocardial infarction has occurred, the ECG is sent to the catheterization laboratory immediately after the ECG is done. Because of this procedure, many people have been resuscitated. Stenting is widely used around the world Is cardiac stenting an outdated technology or not? Dr. Cui Song said that cardiac stent surgery is a more mature technology carried out in the last 20 years to improve the insufficient blood supply to the heart muscle caused by coronary heart disease and the blockage of heart arteries, and has gone through the development of metal stents, drug-eluting stents and soluble stents. Currently, cardiac stent surgery is widely used worldwide. In the United States alone, there are more than 1 million cases of cardiac stenting each year, and in 2012, there were more than 400,000 cases of cardiac stenting in China. Clinical and research studies have proven that stenting is currently a very effective rescue measure for patients with acute heart attacks and other acute coronary syndromes. In addition, the cost of cardiac stenting in the United States is much higher than that in China. The cost of heart stent surgery is mainly two pieces: one is the cost of the stent itself, which is divided into domestic and imported, and there is a price gap, but it is narrowing; the other is the cost of surgery, like the cost of one surgery by American doctors, which usually costs more than 2000 US dollars. If we look at the cost of including the whole stent surgery, it is cheaper in China than abroad. It is understood that one of the risks of heart stent surgery is that after using stents to unblock blood vessels, a small number of cases will be stenosed again at the place where the stent is put, and very few cases will form thrombus, and once the thrombus is formed it will be potentially fatal, but it is proved that the chance of forming thrombus after installing heart stents is less than 1%. Interventional treatment for patients with coronary heart disease accounts for 20% In the rumor, questioned the abuse of cardiac stents in China, for which China has strict regulatory measures, hospitals are required to report the relevant information to the State Ministry of Health and provincial health administrative departments within 10 working days after completing the treatment of each case of interventional treatment for cardiovascular disease. Not every patient with coronary heart disease is suitable for heart stents, and about 20 percent of patients really need interventional treatment. Patients with acute heart attack are the ones who should most urgently undergo stent surgery. Without stents, the mortality rate is about 15%, which can be reduced to 3% to 6% after doing so, and heart stent surgery has saved the lives of many people. Secondly, high-risk patients with significant myocardial ischemia confirmed by examination also need stenting. For some patients with relatively stable coronary heart disease and lesions, doctors will decide whether stenting is needed based on a combination of the condition and examination results.
