In the non-rapid eye movement period, the muscle tone is reduced, there is no obvious eye movement, and the EEG shows slow and synchronous, and when awakened in this period, sleepiness is felt. In the fast eye movement phase, muscle tone is significantly reduced, rapid horizontal eye movements occur, and the EEG shows a state similar to that of awakening.  It has been found that the caudal part of the brainstem has a very important relationship with sleep and is considered to be the location of the sleep center. Various stimulating lesions in this area cause excessive sleep, while destructive lesions cause reduced sleep. It has also been found that central neuromediators are involved in sleep, and stimulation of 5-hydroxytryptaminergic neurons or injection of 5-hydroxytryptophan acid produces non-rapid eye movement phase sleep, while administration of 5-hydroxytryptophan antagonists produces reduced sleep. With norepinephrine antagonists, there is a decrease in REM sleep, whereas administration of norepinephrine agonists produces an increase in REM sleep.  Clinical manifestations 1. The abnormal amount of sleep can include two categories: one is the excessive increase in sleep, such as narcolepsy or somnolence caused by various encephalopathies, endocrine disorders, metabolic abnormalities, and episodic sleeping sickness caused by brain lesions, which is manifested by frequent irresistible sleep episodes of short duration (usually less than 15 minutes), often accompanied by symptoms such as falling, sleep paralysis and hallucinations before falling asleep. Another type of insomnia is sleep deprivation, where the whole night sleep time is less than 5 hours, which is manifested by difficulty in falling asleep, light sleep, easy waking or early waking, etc. Insomnia can be caused by external environmental factors (too much light in the room, too much noise around, night shift, car and boat, just arrived in a strange place), physical factors (pain, itching, violent cough, drinking strong tea or coffee before bed, frequent night urination or diarrhea, etc.) or psychological factors (anxiety, fear, excessive thoughts or excitement). Some diseases are also often accompanied by insomnia, such as neurasthenia, anxiety, depression, etc.  2. For example, sleepwalking and night terrors, mostly occur in the late stage of orthophasic sleep; while somnambulism is mostly seen in the middle or even the early stage of orthophasic sleep; teeth grinding, involuntary laughing, muscle or limb jumping, etc. are mostly seen in the early stage of orthophasic sleep; and nightmares are mostly seen in the period of heterophasic sleep.  In recent years, many doctors from the clinical reality, to explore the mechanism of its occurrence, put forward some new identification ideas.  Some scholars believe that the etiology and pathogenesis of insomnia are mainly manifested in the liver, spleen and the five viscera, and the evidence of the five viscera is considered as a whole. They advocate the holistic view of “all the five organs have insomnia”, the treatment system of treating from liver, taking into account other organs and identifying evidence and adding and subtracting, and thus formulated the treatment plan of insomnia by organs.
