Acute suppurative otitis media is an acute purulent inflammation of the middle ear mucosa caused by bacterial infection. The disease is most often seen in children. It is characterized clinically by ear pain, pus flowing from the ear, tympanic membrane congestion and perforation. If the treatment is timely and appropriate, and the secretions drain freely, the tympanic membrane perforation can mostly heal on its own after the inflammation subsides, and hearing can mostly return to normal. If the treatment is inappropriate or serious, the tympanic membrane perforation, middle ear adhesions, sclerosis of the tympanic chamber or chronic suppurative otitis media may remain or even cause various complications. The treatment of acute suppurative otitis media is generally based on the following methods: 1. Antibacterial and anti-inflammatory The first choice is penicillin or cephalosporin antibiotics intravenously. And can be combined with levofloxacin or erythromycin oral. Or choose antibiotics according to the drug sensitivity test. The dose should be sufficient, the course of treatment is long enough, generally used until the pus and tympanic membrane congestion disappeared. If the effect of using western medicine for more than three days is not good, you should consider switching to other antibiotics, or adding Chinese medicine treatment. (2) External treatment method (1) Nasal drip method: When the disease first starts, use 1% ephedrine saline nasal drip to facilitate smooth drainage of the eustachian tube; (2) Cleaning method: When pus flows from the ear canal, 3% hydrogen peroxide or boric acid water should be used to thoroughly clean the pus from the external ear canal and wipe it dry. (3) Ear dropping method: If the ear is in severe pain and the tympanic membrane is not perforated, phenol glycerin can be put into the ear to relieve the pain. If the tympanic membrane is perforated and overflowing with pus, apply ear drops such as oxyfloxacin (Telbital) or compound neomycin ear drops 3 to 5 times a day; (4) Ear blowing method: If the tympanic membrane is perforated and the ulcer is not smaller than the size of a green bean, blow in powdered medicine 1 to 2 times a day; (5) External application method: If the mastoid area behind the ear is painful or even red and swollen, apply Zijin ingots locally or vinegar mixed with Ruyi Jinhuang San externally. 3.Surgical treatment Tympanotomy – when the general and local symptoms are serious, the tympanic membrane is obviously bulging, and the effect of conservative treatment is not obvious; although the tympanic membrane has been perforated, but the perforation is too small, and the drainage of secretions is not smooth; suspected complications may occur, but there is no need to perform mastoid opening immediately. If mastoiditis has occurred, simple mastoid chisel opening should be performed at the right time to completely remove the diseased tissue and make the drainage unobstructed to prevent complications.
