Endometriosis is characterized by recurrent and progressive dysmenorrhea, heavy menstrual flow, pelvic masses, infertility and other symptoms, which are classified as “dysmenorrhea”, “obstruction” and “infertility” in traditional Chinese medicine. “Modern Chinese medicine treats this disease from blood stasis and has achieved certain clinical and scientific results. However, the one-sided emphasis on blood stasis may be theoretically misleading to our complete understanding of the disease. Combined with the previous discussions on severe dysmenorrhea, we believe that the deficiency of spleen and kidney yang energy and cold-dampness paralysis of the flushing can better reflect the essence of the pathological changes of endometriosis and better help to grasp the dynamic process of the changes of the disease, and we would like to briefly elaborate on the relevant mechanism here, hoping that our colleagues will be correct. 1, the ancient medical theory of cold and wet dysmenorrhea Women’s menstrual water will come three or five days ago, and pain under the umbilicus, like a knife stabbing, or cold and heat, the next as black bean juice, people do not think that the extreme blood heat, who know is the lower jiao cold and wet competition is the reason, the cold and wet is also evil, the woman has the pulse of the lower jiao, Chong for the blood sea, Ren main twins for the blood room, are happy to be connected, the most evil offenses, the menstrual water from the two meridians and out, and The cold dampness is full of the two meridians and internal chaos, and the two compete with each other to make pain, is also the sign of cold water in the north. The treatment: the wet and warm its cold, so that the Chong Ren without the chaos of evil, the umbilicus from the pain of the guilt carry on, the formula with warm umbilical soup: white two, white Poria three money, yam five money, Ba Ji meat five money, lentils three money, white fruit ten, Jian Lianzi three. The evil of cold and dampness is an important causative factor in dysmenorrhea, as cold leads to constriction of the pulse pathways, and dampness is heavy and sticky, easily blocking the qi flow, thus leading to poor menstrual flow and painful obstruction. Although we cannot conclude the relationship between dysmenorrhea and endometriosis as discussed by our predecessors, from the high incidence of endometriosis and the repeated aggravation of dysmenorrhea, we can be sure that the dysmenorrhea mentioned by the ancients should include this disease. 2, the relationship between cold and dampness and stagnation Endometriosis shows recurrent episodes of lower abdominal pain, the disease is located in the lower jiao, lingering and difficult to heal, and other symptoms, consistent with the pathological mechanism of the evil of cold and dampness is easy to block the qi, easy to tend to the yin position, easy to hurt the depletion of yang energy. The recurrent attacks of pain should belong to a long-standing disease. Cold and dampness, together with qi deficiency and qi stagnation, are important factors inducing blood stasis, and at the same time, they are also important factors causing pain. The theory of dysmenorrhea caused by blood stasis, which is generally accepted nowadays, is more common in modern treatises, perhaps as a result of the progress of research methods and research tools in Chinese medicine. However, we should note that blood stasis is a substance with special pathogenic characteristics that appears in the process of disease development, and the occurrence of blood stasis in general is also related to the relative smoothness of the channels. They have different ability to run qi and blood, and the result of interference by external evil is also different, the higher grade, rich in qi and blood, of course, the degree of influence is lighter, while those small Sunluo may show congestion of blood flow, or even occlusion. Microcirculation and other methods should be the simplest and most direct way to observe the stagnation of the ligaments. On the contrary, because of the cold and dampness paralysis of the cellular vasculature and cellular collaterals, during the intermenstrual period, when the uterus is mainly hidden, the blood and fluid can slowly infiltrate it, while after the onset of menstruation, the uterus is mainly diarrhea, its potential is relatively urgent, and the discharge of blood from the cellular vasculature and cellular collaterals is obstructed, resulting in blood stasis, and even the collaterals break the blood overflow and become the blood of menstruation. The blood of menstruation is obstructed, resulting in blood stasis, and even the blood of menstruation breaks down and overflows into the blood of menstruation. 3, the etiology of cold and dampness for the outline of the pathogenesis of cold and dampness as an external or internal evil, the cause of the disease lies in the Yang deficiency, especially the Yang of the spleen and kidney: the kidney Yang contains the fire of the phase or the fire of the vital gate, can warm the internal organs, so that the normal function of the internal organs; the spleen is the essence of the latter, with the function of ascending and clearing, transporting, and consolidating, the spleen Yang deficiency of fluid, Qi, blood transformation is unfavorable, water and fluid metabolism is out of balance. Generally speaking, menstruation, postpartum cell veins empty, if the regimen is unfavorable, feel the cold, or rain and water, or live in the cold and wet, injury to Yang, cold and wet accumulation; or innate lack of endowment, the viscera meridians, blood, essence, fluid loss of warming, Yang deficiency is dirty cold, the loss of Yin is wet. The combination of internal and external evil, long living in the punching of the uterus, resulting in the disease, cold and damp obstruction of the qi machine resulting in qi stagnation, cold and damp stagnation over time into heat, resulting in damp heat. Modern research has confirmed that endometriosis patients have abnormal gene expression, a tendency for the onset of endometriosis to cluster in families, specific adhesion, invasion and angiogenic ability of in situ endometrium, and internal flow of menstrual blood may be the main cause, however, 70%-90% of women of childbearing age have internal flow and only 10%-15% develop the disease. Adhesion-invasion-angiogenesis is the pathophysiological process identified in the formation of endometriotic lesions. Endometriosis is an autoimmune disease.
