The treatment of acne is both simple and complex. Simple because the lightest acne can basically be left to its own devices. Of course, this only means that there is no need to take medication and apply it, but we still have to follow the doctor’s instructions (precautions); complicated because the treatment of heavy acne is no less complicated than that of internal diseases. The treatment of severe acne is three-dimensional and comprehensive: medication, injections, medication, and possibly “baking” (physical therapy), plus various requirements for diet and living. The number of acne treatments is dizzying. New treatments have emerged in recent years. We will try to make a clear and concise introduction to the following: 1. Internal medicine (1) antibacterial drugs, tetracycline, erythromycin, clarithromycin, etc., for pustules and papules as the main damage; (2) lipid suppressants and drugs to promote normal keratinization, the old drugs vitamin A, D (cod liver oil) has been used less, replaced by new drugs retinoic acid, of which 13-cis retinoic acid (isomeric retinoic acid) is the first. Some people say that retinoic acid is an all-purpose acne treatment because it can inhibit lipids, correct abnormal keratinization (follicular sebaceous gland channels) and antibacterial; (3) anti-inflammatory drugs, such as corticosteroids, aminophenyl sulfone (DDS), meclizine, herbal salvia tablets or injections, etc. Corticosteroids are mainly used for severe acne, such as nodular cystic type and acne hypertrophic scars, the former orally, the latter focal injection (i.e., local seal); (4) anti-androgen drugs, the old drug hexestrol (ethinyl estradiol) is easy to obtain, with few side effects, and inexpensive, some of the new drugs are oral contraceptives, and some are new strong estrogen derivatives, the varieties are changing day by day. Some of these drugs are not easily available, and some have large side effects or are not easy to master in terms of usage. They should be used under the guidance of a physician. 2, topical drugs The above internal drugs, there are some available for external use, such as tetracycline, erythromycin, clarithromycin, methotrexate, vincristine, etc.. The older generation of topical drugs such as compound sulfur lotion, 5% sulfur ointment, 20% sodium thiosulfate solution, 10% ichthyolite ointment, etc., are still of practical value. However, some drug formulations are too dry or too greasy and not very applicable, need to be improved. The newer topical drugs are 2-5% benzoyl peroxide (acne vulgaris) ointment, cream or coating agent, which are more popular. This drug is both bactericidal and lipid inhibiting, killing two birds with one stone. 3.Physiotherapy The old generation of ultraviolet light and superficial radiation therapy has “fallen out of favor”. The new generation of low-power lasers such as helium-neon laser and liquid nitrogen freezing are yet to be observed for sure. 4.Other Inverted mask treatment is a “newcomer” and is applicable to all stages of acne, but the primers used should vary depending on the condition. It is contraindicated in cases of inflammation and pus discharge. This method is highly technical and must be operated by an expert. Simple acne can be squeezed out with a special acne squeezer. Nodules and scars can be closed with hormones. 5.Chinese medical treatment Chinese medicine believes that this disease is mostly caused by unclean diet, damp heat in the lung and stomach, and poisonous evil. The treatment is to clear the damp-heat in the lung and stomach, cool the blood and detoxify the toxin. The disease is often treated by adding and subtracting the batch of Lo Pa Ching Lung Drink, and adding and subtracting Gardenia Gold Flower Soup for secondary infection, and using Tao Hong Er Chen Tang for the formation of nodules after a long time. When the rash is red and swollen during the acute attack, first use mulberry bark, ground bark and scutellaria to clear lung heat; Huang Lian and gardenia to clear stomach heat, gardenia also to clear the real fire of the three jiao; double flower, forsythia, dandelion and purple flower ditin to clear heat and detoxify; peony bark, red peony and summer cress to cool the blood and detoxify softness; carthamus seed and barley to clear damp heat; cocklebur and acacia rice to cool the blood and clear heat. If itch is heavy, add white ringworm bark and bitter ginseng; if sebum overflows, add raw white atractylodes, raw barley and raw hedgehog shell; if stool is dry, add guajia and Chuanjun; if women have irregular menstruation, add salvia, aromatic herb and Kuncao; if inflammation subsides and nodular cysts are left with powder, the main purpose is to regulate qi and invigorate blood, soften and disperse nodules, and use Xia Ku Cao, ghost shear feather, salvia, safflower, trigonella, curcuma longa, keel, oyster, aromatic herb, hedgehog shell, Chen Pi and Semen.
