Combined Chinese and Western medicine treatment of hemorrhoids

Hemorrhoids are one of the most common and unique human diseases in anorectal surgery, both men and women are at risk of developing them, and their incidence accounts for about 85% of all anorectal diseases, so the study of hemorrhoids is a hot topic of concern. Most people lack knowledge about hemorrhoids, and it is difficult to talk about such diseases, and the early symptoms are not paid attention to, so the prevalence of hemorrhoids is getting higher and higher. We have achieved good clinical results. The combination of Chinese and Western medicine in the treatment of hemorrhoids in our hospital has achieved good results, which are summarized as follows. (1) Chinese medicine internal treatment Chinese medicine attaches great importance to the application of internal treatment for hemorrhoid disease, which is characterized by different types of identification and treatment, and we classify hemorrhoids into four types of evidence (wind injury to intestinal ligaments, damp heat injection, qi stagnation and blood stasis, spleen deficiency and qi trapping) for treatment. Wind injury to intestinal ligaments: Treatment: clearing heat and cooling blood to dispel wind. Prescription: Cool Blood and Dihuang Tang plus or minus. Chinese patent medicines: ①Mahren Pills, 2 times daily, 6 grams each time with warm water. ② Hemorrhoid Kang tablets, 3 times a day, 3 tablets each time with warm boiled water. Damp-heat infusion type: Treatment: Clear heat and dampness to stop bleeding. Prescription: Gentian and liver diarrhea soup plus reduction. Chinese patent medicine: 1. Doulian Pill, 1 packet twice a day with warm water. Qi stagnation and blood stasis: Treatment: clear heat and dampness, dispel wind and invigorate blood. Remedies: pain relief like Shen Tang plus or minus. Chinese patent medicine: Blood stagnation and blood stasis capsule, 6 capsules twice a day with warm boiled water. Spleen deficiency and Qi trapping type: Treatment: tonifying the middle and benefiting Qi and raising it. Chinese herbal medicine: tonifying Zhong Yi Qi Tang. Proprietary Chinese medicine: tonifying Zhong Yi Qi Pills, 8 capsules twice daily, taken with warm boiled water. After surgery, we used Mu Xiang Tong Li Tang to treat common complications after PPH surgery such as pain, bleeding, urinary retention and cramping with good results. M): 4 tablets taken orally three times / day, is an effective and safe drug for the treatment of hemorrhoids. ② Diosmin sign: 2 tablets taken orally three times/day, also commonly used in the treatment of hemorrhoids. Non-specific drugs: commonly used are: ① pain relievers: acetaminophen, acetylsalicylic acid, anandamide, etc. ② stool softening agents: hemp seed pills 6 grams orally twice / day, Fosone (polyethylene glycol 4000 dispersion) 1 packet orally twice / day. ③ Hemostatic drugs: Yunnan Baiyao, vitamin K, or Anluo blood and other hemostatic drugs, or intramuscular injection or intravenous injection of hemostatic min, hemostatic aromatic acid drugs. 2, external treatment method is mainly divided into fumigation sitz bath, external application, acupuncture, plugging medicine, etc., the current clinical more commonly used several therapies are: ① fumigation sitz bath method: we use the empirical formula fumigation combination (bitter ginseng 20g, rhubarb 20g, cypress 20g, gentiana 20g, white fresh skin 20g, ice chips 3g, wubaizi 20g, angelica 20g, frankincense 20, mannite 30g), we have conducted a controlled study The efficacy of the herbal fumigation combination in treating post-operative anal edema and pain complications after PPH is remarkable. ②External application method: Ma Yinglong hemorrhoid cream or anal tide ointment is used externally, and for acute onset of edema and pain hemorrhoids are applied externally with borax san (borax, ice chips, mannitol, etc.), which is effective. ③ Plugging method: commonly used suppositories such as Puji hemorrhoid suppositories, anal tai suppositories, taining suppositories, etc. 3, acupuncture: PPH spasmodic pain in the lower abdomen, postoperative urgency, urinary retention, anal pain, etc. are more common complications, we use acupuncture to prevent PPH intraoperative and postoperative adverse reactions, and achieve better results. 4.Glue ring ligature therapy: This method is still an effective therapy between injection therapy and surgical therapy, which is simple, cheap, safe and effective. 5.Anal expansion therapy: mainly to cut off the hemorrhoid block to form a vicious cycle of “congestion, obstruction and congestion” therapy, usually used for the treatment of I-II internal hemorrhoids, manipulation of anal expansion is easy to operate, does not require too much training or special equipment, the effect is particularly good for patients with anal canal hypertension, but anal expansion is only applicable to patients with pain and bleeding hemorrhoids, not for patients with prolapse or overflow. Especially in the elderly, dilation to 6-8 fingers can traumatize the anal sphincter and make it easy to complicate anal incontinence, which can persist and be difficult to treat.
