Management of iliac artery rupture during iliac arterioplasty

[Abstract] Objective To investigate the value of overlapping stents in the application of iliac artery rupture encountered during iliac arterioplasty. Methods Retrospective analysis of the clinical data of 9 cases of iliac artery rupture during endovenous iliac artery angioplasty treated with overlapping stents from January 2010 to January 2012 in the Department of Vascular and Endovascular Surgery of the First Affiliated Hospital of Zhengzhou University. The results showed that all 9 patients had successful placement of the overlapping stent, 8 patients had immediate review of the iliac artery angiogram, which showed that the lumen of the iliac artery was patent and there was no spillage of contrast agent; 1 patient had type I endoleak at the proximal end of the overlapping stent, and “iliac artery artificial vessel replacement” was performed immediately by open abdomen. One patient had stenosis of the distal end of the overlying stent (external iliac artery) one year after surgery, and balloon dilatation of the external iliac artery with bare stenting was performed, and follow-up was continued for 5 months without adverse events. Conclusion When iliac artery rupture is encountered during iliac arterioplasty, immediate placement of overlapping stents is preferred for treatment, and surgical treatment is the last safeguard.
