Overview of somatoform disorders Definition: a class of mental disorders in which a variety of somatic symptoms are the main clinical manifestations, and in which no organic damage or clear pathophysiological mechanisms can be proved, but in which there is evidence of a close relationship with psychological factors or inner conflicts. Somatoform disorders subtypes 1, somatization disorder 2, hypochondriasis 3, somatoform autonomic dysfunction 4, somatoform pain disorders Somatization disorder 1, the main characteristics of a wide variety of, recurrent, often changing somatic symptoms, sometimes there are simulated neurological symptoms, the patient repeatedly complains of variable somatic symptoms, which can be related to any system and any part of the body, and often exaggerated. The most common symptoms are: gastrointestinal sensations (pain, hiccups, acid reflux, vomiting, nausea, etc.), abnormal skin sensations (itching, burning, tingling, numbness, soreness, etc.). 3. Accompanied by obvious anxiety and depression. Hypochondriasis 1. The main characteristic of hypochondriasis is that the patient is excessively worried about his/her own health or disease, and he/she is afraid that he/she is suffering from some kind of disease or he/she thinks that he/she has already suffered from a serious disease, and he/she feels very annoyed. The severity of the worry is very disproportionate to the patient’s actual health condition. 2, some hypochondriacal discomfort is very obvious, can be accompanied by anxiety or depression (sensory hypochondriasis); some hypochondriacal concepts are more prominent, physical discomfort, or changes in state of mind are not significant (conceptual hypochondriasis). 3, body dysmorphic disorder and disease horror belongs to the category of hypochondria. Somatic form of autonomic dysfunction 1, the main characteristics of the autonomic excitation with obvious symptoms (such as palpitations, sweating, blushing, tremor), but also non-specific symptoms attached to the subjective complaints, such as the site of the indeterminate pain, burning sensation, heaviness, tightness, swelling. The most common and prominent cases involve the cardiovascular system (“cardiac neurosis”), the respiratory system (cardiac cough with hyperventilation) and the gastrointestinal system (“gastric neurosis”, “nervous diarrhea ” and ” irritable bowel syndrome “). Somatoform pain disorders 1. The main feature is a prominent complaint of persistent, severe, excruciating pain that cannot be fully explained by physiological processes or somatic disorders. 2. Emotional conflicts or psychosocial problems are often the main causative factors. Treatment of somatoform disorders 1, such patients are often accompanied by depression and anxiety, but also somatization complaints, the use of SSRI (selective 5-hydroxytryptamine reuptake inhibitor) class antidepressants or other antidepressants with fewer side effects and a significant anxiolytic effect is also valuable. 2, Cognitive behavioral therapy and SSRI antidepressants in combination are considered more effective than drugs alone or cognitive behavioral therapy alone. 3, Amitriptyline can be used in the treatment of somatoform disorders, but its cardiovascular side effects need to be concerned.
