Treatment of thrombotic thrombocytopenic purpura plasma replacement is the first choice, followed by plasma transfusion, which can lead to remission in 64% to 80% of patients. The replacement fluid is mainly fresh frozen plasma and should be performed as early as possible, preferably within 24 hours of onset.
Replacement is performed once daily during the first period and then every other day or twice weekly after 3 to 4 days until platelets are greater than 150 × 10⁹/L, the patient’s condition is in remission, neurologic symptoms have improved, serum lactate dehydrogenase levels are near normal or normal, and hemoglobin values have risen.
The diagnosis of thrombotic thrombocytopenic purpura should be followed by early and standardized treatment to minimize the adverse effects of the disease.
