The repair of jaw defects is a serial treatment process, and the final result of defect repair depends on the whole process rather than just the final pseudo-restoration. (i) Preoperative design and preparation 1. The preoperative maxillofacial surgeon and the prosthodontist consult together to develop the surgical plan, determine the scope of surgery, the preserved structure and the surgical method. 2. After the surgical plan is determined, the prosthodontist makes an impression of the patient and trims away the corresponding parts of the model according to the predetermined surgical plan to create a preformed palatal plate for the patient. (B) Intraoperative application of palatal plate After maxillary resection, after hemostasis and/or skin grafting, local dressing and pressure dressing can be applied, at which time the palatal plate is placed on the patient. The main purpose of the palatal shield is to (1) stop the bleeding by filling the palatal shield with iodoform gauze and dressing, and to put pressure on the intraoral wound. (2) To protect the surgical area from contamination by saliva and food. (3) To help eating and reduce the patient’s psychological burden. (4) Reduce scar contracture and reduce facial deformity. (3) Postoperative temporary obstruction device (interim obturator) wear 1, 1-2 weeks after surgery, remove the palatal guard plate to clean the trauma. 2.The palatal shield is converted into a temporary obturator. Usually, self-consolidating resin is used layer by layer over the palatal guard type denture, which is shaped directly in the patient’s mouth so that the part of the obturator entering the defective cavity fits perfectly with the tissue of the defective area. 3. During wear, discomfort may occur due to local scar contraction, which may be abraded or lined. (D) permanent prosthesis (definitive obturator prosthesis) wear about 3 months after surgery, the defect area of the trauma has basically healed, inflammation has disappeared, the surrounding tissue tends to stabilize, at this time that the patient should be made permanent prosthesis, restore the patient’s lost speech, chewing, swallowing and other functions, restore the patient’s facial appearance.
