Why is fungal bulb-type maxillary sinusitis difficult to cure?

Patient Question: Disease: left upper frontal sinusitis has not been well content; Description of the disease (onset, main symptoms, hospital visited, etc.): Endoscopy was done with the left nasal polyps/open upper frontal sinus and a small opening in the lower nasal tract to clean up the stuff at the bottom of the upper frontal sinus, and now that small opening has healed. Previous treatment and results: The results were not good. After the operation, I had regular nasal endoscopic cleaning, but it was probably due to the equipment and technique of the clearing staff, and it was never cured. Now the nasal cavity is every few days there will be a block like bean curd-like yellow pus nasal discharge in the throat, and then to Guangzhou doctor cleaned up three times, nasal endoscopy to see a large number of sinus pus crust, but after treatment or every ten days or so, there is still a round hard yellow-green pus crust from the nose or throat, and now the nose according to the discomfort, wearing glasses will be uncomfortable, head some dizziness call. I think I must not have found a doctor who can cure this disease. Please teach me how to confirm that my disease is fungal, and how to cure my left upper frontal sinus, which is open. Thank you. (The attached file is the last postoperative review CT, please refer to it, thank you) Reply from Wang Xianzhong: Are you in mainland China? It’s such a simple and clear disease, but it takes two years to get rid of it. The best medical resources in mainland China are concentrated in 4 centers: Beijing, Shanghai, Guangzhou and Chengdu. You have been to one of them. How can you explain what happened to you? Patient Question: Today I went to see a professor in the treatment of sinusitis at the Guangzhou Provincial People’s Hospital, and he said it was normal to have scabs, and the more surgery I had, the more scabs there would be, saying that the mucous membranes in my sinuses were gone, and that it could be the surgery or the erosion of secretions. I said that then I have discomfort on the side of my nose when I press it, and it is uncomfortable when I wear glasses. He said that this could be trigeminal neuralgia, and that I might have injured the trigeminal nerve during the surgery. He also told me to find the CT before the first surgery, but it was gone because of the move, so I had to go to the hospital where the first surgery was performed to find it, but what if the information cannot be found after many years? I feel that my condition is getting more and more complicated. What should I do? Wang Xianzhong replied: The trigeminal nerve has three branches as the name implies, did the expert professor tell you which one was injured? There is obviously a high-density shadow in your maxillary sinus, and the bone of the sinus wall is obviously thicker than the opposite side. These are the typical CT imaging manifestations of fungal bulb type maxillary sinusitis. You said that this picture was the last CT picture. Then, according to this picture, there is still a possibility of fungal masses surviving in your maxillary sinus, which must be surgically cleared again under general anesthesia. The only way you can heal is to clean out the fungal mass in the maxillary sinus. Is the problem so simple that it needs to take two years to solve? If you live in a developed European capitalist country, this is indeed a possibility. However, you are in mainland China. The motivation of the doctors in mainland China is the best in the world! The surgical skills are the best in the world! Wang Xianzhong replies: There are many things in the world that should be done well but are not done well. There are things that can’t be done, and there are things that you want to do but can’t. Every doctor hopes that he or she can get rid of the disease or hand to hand, to relieve the patient’s pain and make a name for himself or herself. Then you know how you should do it.
