For people who were healthy in the past, they should be alert to bone metastasis of tumor for symptoms such as back and leg pain, severe anemia, etc. The distant metastasis of malignant tumor is the malignant sign and characteristic of tumor and one of the main reasons for treatment failure of tumor patients, bone tissue is the third most common organ for distant metastasis of cancer, after liver and lung. Theoretically, every malignant tumor can metastasize to the bone, among which breast cancer, prostate cancer and lung cancer are more common. As we all know, early detection and treatment of malignant tumors are very crucial to the healing process. In our life, some patients have symptoms such as back and leg pain or anemia (especially those who have a history of such diseases, such as rheumatism, lumbar disc herniation, etc.), but they do not pay enough attention to them, in fact, it is likely that this is a precursor of tumor bone metastasis. I will give you 2 real cases: a 66-year-old male patient, who was admitted to our orthopedic department in 2006 with “lumbar disc herniation” due to 5 months of lumbar pain and 3 months of aggravation, had been seen in many hospitals before that, had taken X-ray of lumbar spine, no bone destruction was found, and had been treated according to “lumbar disc herniation”. “In September 2006, he was first admitted to the orthopedic department of our hospital with “lumbar disc herniation” and was diagnosed as “bone metastasis of lung cancer” after careful examination. After careful examination, he was diagnosed as “bone metastasis of lung cancer” and then transferred to the Department of Oncology for treatment, and the situation at that time was: multiple metastases all over the body and limited activities. This case gives us a hint: when there is back and leg pain, especially for the elderly, do not rush to do massage and massage treatment first, but must check and confirm the diagnosis, especially be alert to tumor bone metastasis. A 33-year-old female patient with intermittent gastric and epigastric discomfort for 3 years was not taken seriously. After treatment, the disease is now stable and is still in hospital for consolidation treatment. This case gives us a hint: unexplained severe anemia must be examined and diagnosed, especially to be alert to tumor bone metastasis. Bone metastasis is one of the common complications of malignant tumors, and bone metastasis occurs in one out of every five patients on average. The quality of life can be seriously affected by severe pain (example 1, cervical cancer fracture on turning, example 2, prostate cancer fracture on bathroom). Many patients and their family members think that there is no good method after the development of bone metastasis, and they can only use morphine and other drugs for pain treatment. In fact, patients with bone metastases still have a long survival period, and through reasonable treatment of both symptoms and root causes, many patients with bone metastases can stand up again and live and work painlessly like ordinary people. Early diagnosis The early diagnosis of bone metastasis is very important, and it can be said that the earlier the diagnosis is confirmed, the more favorable the treatment is. We often encounter such cases in the clinic. Patients who suffer from accidental fall or sprain are found to have primary bone tumor or bone metastasis only when they go to the hospital for examination. The cases that are more likely to be misdiagnosed clinically are those patients who do not see any obvious abnormality after fall or sprain by ordinary X-ray examination, because the positive diagnosis rate of bone metastasis by X-ray light film is relatively low (about 57%). The main symptom of bone metastasis cancer is persistent pain with persistent aggravation, which may also cause activity impairment. Therefore, patients with diagnosed malignant tumors should be reviewed regularly to be alert to the occurrence of bone metastases. For patients with common trauma, if the symptoms persistently cannot be relieved, further examination should be conducted to clarify the diagnosis. Do not easily carry out massage and other treatments before a clear diagnosis is made, so as not to aggravate the condition. The commonly used clinical imaging methods for bone metastasis diagnosis have different characteristics: X-ray has high specificity but low sensitivity; bone ECT (bone scan) has a high positive rate, but there are problems of false positives and false negatives; CT and MRI (magnetic resonance imaging) have high specificity and accuracy, but are not suitable for whole body examination; PET (positron emission computed tomography) has a high positive rate, but is less effective in simple PET (Positron Emission Computed Tomography) has a high positive rate, but is less effective and relatively expensive for purely osteogenic lesions. Theoretically, the alteration of biochemical indicators of bone metabolism in bone metastasis precedes imaging, but the lack of specificity of traditional biochemical indicators of bone metabolism and interference by various factors limit its application in bone metastasis research. Some new biochemical indicators of bone metastasis have good diagnostic value, such as type I collagen carbon-terminus peptide (ICTP): sensitivity, specificity and accuracy reach: 92.05%, 70.0% and 81.6%, respectively. Bone metastases are manifested in three types: osteolytic, osteogenic and mixed. The treatment methods are different for different types. Only with clear diagnosis and symptomatic treatment can we have good clinical effect. Therefore, the clinical efficacy of single method of treatment is not good. Relying on the advantage of our hospital as a national orthopedic medical center, the Department of Oncology of our hospital has developed a comprehensive treatment method for tumor bone metastasis through the summary of past experience and exploration of new diagnosis and treatment methods. In the comprehensive treatment of tumor bone metastasis, we have developed the “Balanced Blocking” therapy with the characteristics of combining Chinese and Western medicine, and achieved very good results in clinical practice. The connotation of “balance blocking” therapy is based on the principles of “balance of yin and yang”, “prevention before illness”, “prevention of change after illness”, and “kidney is the mainstay of bone metastasis” in Chinese medicine. Based on the theory of “Yin and Yang balance”, “prevention before disease”, “prevention before change”, “prevention after disease”, “the kidney is the master of bone and marrow”, and modern molecular biology theory of tumor bone metastasis, “balanced blocking” therapy for bone metastasis is proposed, the core contents of which are The core contents of the therapy are: (1) Early diagnosis, early detection of bone metastasis, and evaluation of the treatment effect of the commonly used clinical imaging methods, such as X-ray, CT, ECT (bone scan), MRI (magnetic resonance imaging) and PET (positron emission computed tomography). Theoretically, the change of bone metabolism biochemical indexes in bone metastasis precedes imaging, so we have carried out tumor marker and bone metastasis specific sensitive index serum test in clinical practice to scientifically detect the development of tumor, residual after surgery and postoperative recurrence. In terms of bone metastasis specific sensitive index serum test, we have carried out osteogenic metastasis index: blood bone specific alkaline phosphatase (BALP), blood type I collagen pre-carbon terminal peptide (PICP). For osteolytic metastases: blood type I collagen carbon-terminus peptide (ICTP). Bone metastasis specific sensitive index serum test, in the clinical bone metastasis changes 3-6 months earlier than the imaging changes, also help to analyze the type of bone metastasis, according to different metastasis types, the evidence-based treatment, can achieve better clinical efficacy. (2) Reducing tumor and eliminating evil Using minimally invasive, nuclear, radiotherapy and chemotherapy to reduce the tumor load, improve the immunity of the body, and achieve the immune balance between the body and the tumor in modern medicine. The Chinese herbal medicines with positive and anti-cancer effects are applied simultaneously to achieve the balance of positive and evil, yin and yang between the body and the tumor in traditional Chinese medicine. (3) Interrupting metastasis According to the law of metastasis of different tumors, based on the treatment principle of “supporting the righteousness and cultivating the root” of tumors and the method of “nourishing Yin and tonifying the kidney”, we apply the formula “Yi Kidney Bone Health Tang” on the basis of our experience. On the basis of “Yi kidney bone health soup”, the formula is added and subtracted to improve the immunity of the body, block the binding of osteoclasts or osteoblasts produced by the tumor with the receptor, repair the damaged bone tissue, and stop the development of bone metastasis. Bone metastasis cancer belongs to the categories of “bone tumor”, “bone erosion”, “bone gangrene” and “bone paralysis” in Chinese medical literature. In the treatment of bone metastasis pain, TCM generally believes that the kidney is the master of bone and marrow production, and the main pathogenesis of bone metastasis cancer is kidney deficiency, accompanied by blood stasis, etc. Then, we should examine the causes and identify the evidence, and find out the transmission pattern among the five organs according to their respective biases, so as to regulate their bias, supplement their deficiency and prevent their transmission. The latest clinical research shows that osteogenic metastases are mostly kidney yang deficiency, and osteolytic metastases are mostly kidney yin deficiency. For some bone metastases that are not sensitive to drug treatment, minimally invasive interventional treatment – percutaneous vertebroplasty (PVP) – can achieve complete or significant pain relief, provide structural support to the destroyed bone tissue, prevent pathological fractures and paraplegia, and kill local cancer cells within 72 h after surgery, according to this treatment method Based on the advantages and shortcomings of this treatment method, we conducted a deeper study based on the traditional method, and added different anti-tumor drugs to the bone cement according to different primary lesions to further improve the clinical efficacy.
