The majority of esophageal cancers are squamous cell carcinomas, followed by adenocarcinomas, while adenosarcomas and carcinosarcomas of the esophagus are less common. Esophageal cancer mostly occurs in the middle and lower part of the esophagus, and less in the upper part of the esophagus, which is often complicated with esophageal cancer. Diagnosis of esophageal fistula caused by esophageal cancer in Chinese medicine is as follows: 1. Liver depression and qi stagnation: discomfort in the throat, burning pain or foreign body sensation in the sternum when eating, distension and fullness in the stomach and epigastrium, tandem pain in the chest and hypochondrium, and the symptom will be aggravated by emotional and mental disorders, with a dark tongue, thin white moss, and thin stringy pulse. The treatment is to dredge the liver and regulate qi, detoxify and fight against cancer. 2, Internal obstruction of stasis and poison: dysphagia, chest pain, pain in a fixed place, fixed and immovable, aggravated when swallowing in the back of chest and the sword protruding area, dark tongue, or with petechiae, thin or tight pulse, treating with activating blood circulation to remove blood stasis and regulating qi to stop pain. Spleen deficiency and phlegm-dampness: dysphagia, phlegm and saliva congestion, vomiting a lot of mucus when choking on food, chest congestion, distension and fullness of the ribs and chest, facial swelling, yellowish and colorless, dark red tongue, fat tongue with teeth marks, white greasy moss, and slippery or dull and weak pulse. The treatment is to strengthen the spleen and promote dampness, lowering the rebelliousness and resolving phlegm. 4, Yin deficiency and internal heat: dry mouth and tongue, difficulty in swallowing, near obstruction, vomiting, emesis, emaciation, irritable heat and dryness of lips, dryness and knotting of stools, five heartburn and heat, restlessness in sleep at night, red tongue with reddish-red color and thinness, little moss, and fine pulse. The treatment is to nourish yin and clear away heat, detoxify and remove vexation. 5, qi and blood deficiency: swallowing and choking, wasting, pale, fatigue, shortness of breath, palpitation and insomnia, swelling of the lower limbs, abdominal distension and diarrhea. The tongue is pale red, or dry and thin with little moss, and the pulse is dull and weak with heavy pressure. Treatment should strengthen the spleen and vital energy, nourish blood and replenish blood.
