Clinically, sciatica can produce different pain symptoms depending on the location of sciatic nerve involvement, which are called radicular sciatica and dry sciatica. How to distinguish between radicular sciatica and dry sciatica? 1, radicular sciatica lesion site: spinal nerve roots in the spinal canal; common disorders; lumbar disc herniation, lumbar spinal stenosis, spondylitis, spina bifida; pain site: from the waist to one side of the hip, the back of the thigh, lateral calf; pressure points: lumbosacral, the spine has a fixed and obvious pressure pain, percussion pain; sensory impairment: calf lateral dorsal foot sensation is reduced; tendon reflex: knee reflex, Achilles tendon reflex is reduced or disappeared; Increased abdominal pressure: increased pain. Straight leg raise test is positive. 2, dry sciatica lesion site: outside the spinal canal along the sciatic nerve travel; common diseases: sacroiliac arthritis, hip arthritis, hip injury, pelvic inflammation and swelling, pear-shaped muscle syndrome; pain site: low back pain is not obvious, pain along the sciatic nerve travel below the hip; pressure points: between the sciatic tuberosity and the greater trochanter, the central N fossa, under the small head of the fibula; sensory impairment: hypoesthesia of the dorsum of the foot on the lateral side of the calf; tendon reflex: decreased or absent Achilles reflex; increased abdominal pressure: no effect; muscle: weakness or even mild atrophy. Straight leg raise test is positive at 60 degrees, but turns negative after 60 degrees. Whether it is radicular sciatica or dry sciatica, the more common lesion is nerve edema, discharge-like pain and numbness caused by the entrapment of nerve roots or trunks, so the treatment is mainly directed at the entrapment of nerves. The treatment methods of our department for radicular sciatica include lumbar disc radiofrequency, ozone and collagenase, etc.; while the treatment methods for dry sciatica include sciatic nerve injection and pear-shaped muscle intensive silver release treatment, etc.
