Preoperative procedure for Cushing’s disease

  For patients with outpatient consideration of Cushing’s syndrome, they are admitted to the endocrine ward for screening and complete a series of localization and qualitative examinations before being referred to neurosurgery for surgical treatment if Cushing’s disease is considered. These tests include: 1. Routine tests: routine blood, urine, liver and kidney function, coagulation, infection, blood group, electrocardiogram, chest X-ray, etc.  2.Hormonal tests: blood cortisol rhythm, ACTH, 24h-UFC, PRL, GH, IGF-1, LH, FSH, T, E2, etc.  3.Endocrinology experiments: LDDST, HDDST, etc.  4.Imaging: CT of thorax, abdomen and pelvis, CT of adrenal gland, bone density, MRI of pituitary gland, PET-CT, IPSS if necessary. 5.Endocrine check-up, queue ectopic ACTH syndrome, consider pituitary-derived tumor, will ask neurosurgery consultation. For difficult collapse, a multidisciplinary consultation will be requested.  6.After consultation, if surgery is indicated, the patient and family are informed and understand the risks of surgery and are referred to neurosurgery.  7.Since all examinations are completed in endocrine, the patient is usually referred to neurosurgery for 1-2 days for surgery. The patient is discharged from the hospital about 3 days after surgery.
