Abstract: Deviated nasal septum is a deviation of the nasal septum to one or both sides, or a localized bulge, which eventually leads to nasal dysfunction. There are many clinical types of nasal septal deviation, such as S-type and C-type. In this article, we present a 52-year-old male patient who came to the clinic with nasal congestion for 20 years and headache for 6 months. After detailed questioning about the related conditions, the patient was recommended to have a coronal CT of the sinuses, after which the diagnosis of nasal septal deviation was confirmed, and the patient was recommended to undergo surgery, after which the nasal ventilation and headache disappeared.
Basic information】Male, 52 years old
Disease Type】Deviated nasal septum
Hospital】Northern War Zone General Hospital
Date of consultation】May 2021
Treatment plan】Surgical treatment (nasal endoscopy to correct nasal septum deviation) + nasal washers to rinse nasal cavity + intravenous medication (cefuroxime sodium for injection) + oral medication (nifedipine tablets + eucalyptus pinene enteric capsules)
Treatment period】6 days of inpatient treatment, 1 week of outpatient review
Treatment effect] The patient’s nasal congestion and headache disappeared after surgery
I. Initial consultation
After anti-inflammatory and anti-swelling treatment, the patient felt that there was no significant change in the appearance of his nose, so he did not go to the hospital, but since then, he felt that the nasal ventilation was poor, and the symptoms of nasal congestion would worsen after upper respiratory tract infection. However, 6 months ago, the patient started to have headache symptoms, which were aggravated when the nasal congestion was obvious, and she sprayed Tretinoin nasal spray and took oral Tonsure rhinitis tablets, but the symptoms improved when she took the medication, but persisted after she stopped taking the medication. The patient was advised to improve the coronal CT of the nose, which indicated a significant deviation of the nasal septum and suggested surgery to solve the structural abnormalities in the nasal cavity.
II. Treatment history
The patient’s blood pressure was high at the time of admission, so he was given blood pressure monitoring and oral antihypertensive drug nifedipine tablets to control his blood pressure. The procedure was performed under general anesthesia.
III. Treatment results
The patient removed the nasal expansion sponge 2 days after the operation and complained that the nasal ventilation was significantly improved compared with before. During the nighttime checkup, the patient complained of no recurrence of headache, but there was still discharge from the nasal cavity. The patient was discharged after 6 days of hospitalization and was asked to come to the hospital for review in 1 week.
IV. Notes
Although the patient was successfully discharged from the hospital, the treatment is still not over. After discharge, follow the doctor’s prescription for medication and come to the hospital for review after 1 week.
1. continue to rinse the nasal cavity every day after discharge, avoiding excessive force when rinsing to avoid causing nasal septal hematoma.
2.Continue to take oral eucalyptus pinene enteric capsule after discharge, but note that you should not reduce or stop the medication on your own, and should take the medication correctly under the guidance of the doctor.
3. 1 week after discharge, return to our department for a review to clarify the situation in the nasal cavity and prevent adhesions in the operated area.
4, the diet must be light, avoid eating spicy and stimulating food, avoid smoking, drinking alcohol and staying up late.
5, usually also need to exercise properly to avoid upper respiratory tract infection.
6, the condition changes at any time to seek medical advice, please do not deal with their own at home, so as not to delay the disease.
V. Personal insight
Nasal septal deviation is a very common disease in ENT clinic, and a series of clinical manifestations can bring trouble to patients’ life. Therefore, when nasal congestion, headache and other related symptoms appear in life, promptly go to the ENT department of the hospital for consultation, and after diagnosis, consider further treatment options to avoid affecting normal work life and restore normal ventilation function of the nasal cavity as early as possible to avoid complications. Generally, after active treatment, as in the case of this patient, better results can be achieved.
