Concerned about snoring and “impotence”

Snoring, many people may have touched such experience, in your once school, business trip, the people around you like a thunderous snoring seriously affect your sleep, had to get up to wake up, let the snorer side sleep, although you can temporarily reduce the snoring, but a moment will start again. Or, you are the one who snores badly, and you feel anxious about the troubles you bring to the people around you. However, many people think that snoring is a common phenomenon among most men and does not need to be taken seriously. Snoring, generally known as obstructive sleep apnea hypoventilation syndrome (OSAHS), is a common disease that manifests itself as snoring during sleep at night, accompanied by respiratory arrest or hypoventilation, which results in recurrent hypoxemia or hypercapnia in the body. A foreign survey found that 24% of men aged 30-60 had sleep apnea, and 4% of them met the diagnostic criteria for OSAHS. This compares to a preliminary national prevalence of 3.62%. It can occur at any age, with the highest prevalence especially in middle-aged obese men. So, from the perspective of male reproductive health, will severe snoring have a greater impact on this? Could it even lead to “impotence”? The answer is yes. Under normal physiological condition, the production of testosterone in human body is controlled by the regulation of hypothalamic-pituitary-gonadal axis, and the peptidergic neurons in hypothalamus can synthesize gonadotropin-releasing hormone through pituitary portal system into pituitary gland, which can increase the synthesis and secretion of testosterone, and testosterone can regulate hypothalamic-pituitary-gonadal axis by negative feedback. Severe snorers can repeatedly experience sleep apnea during sleep, which can lead to sleep fragmentation and intermittent hypoxia, which can inhibit and damage the hypothalamic-pituitary-gonadal axis, thus disrupting the internal environment of the body and causing abnormalities in the secretion of sex hormones, especially a decrease in testosterone levels. As a biologically active androgen, the decrease of testosterone can directly lead to the decrease of male libido and sexual function, and even the change of personality. On the other hand, repeated hypoxia at night in patients with severe snoring can directly damage vascular endothelial cells and inhibit the synthesis of prostaglandins and nitric oxide, resulting in a series of pathophysiological changes in the penile corpus cavernosum and erectile dysfunction, which is commonly referred to as “impotence” by the general public. The treatment of severe snoring includes general methods such as reducing weight, changing body position, quitting smoking and alcohol, applying oral orthodontic appliances, surgical treatment and continuous positive airway pressure ventilation, among which continuous positive airway pressure ventilation is one of the most effective methods. Therefore, if you have a serious snorer around you, especially if there is a short period of apnea during sleep, for the health of your loved ones and friends, please suggest to seek medical treatment as early as possible.
