Elderly people who cannot take care of themselves, visually impaired people, infants and children under 12 years old, and mentally unsound people should not be irrigated. Patients with stoma prolapse, stoma hernia and other stoma complications should not be irrigated, and cancer patients are forbidden to irrigate during radiotherapy and chemotherapy and within half a year after the end of radiotherapy and chemotherapy. In addition, patients with systemic diseases, such as heart or kidney disease, arthritis, etc., chronic enteritis, diarrhea, weak patients are also not suitable for irrigation. Before learning the method of irrigation, it is necessary to obtain the consent of the surgeon and consult with a professional stoma therapist. It is emphasized that it is best to get demonstration and guidance from a professional for the first two times of irrigation. Lavage can be performed by the patient himself, and requires a set of special lavage equipment (special conical lavage head, which is soft and not easy to damage the mucous membrane of the stoma), a separate toilet, and an appropriate amount of warm water. It is best to have a family member present at the start of irrigation, and the whole process takes about 45-60 minutes. Most of the time is spent waiting for the water to drain out the stool, at which time the stoma patient can watch TV or read books. After the irrigation fluid is drained, the stoma is dried and a piece of gauze is applied. The amount of liquid for each irrigation varies from person to person, ranging from 400-800cc, and when the person who irrigates feels bloated, it indicates that the amount is sufficient. The flow of water during irrigation should not be too fast. Generally at an even flow rate into the water 10-15 minutes can be. The temperature of the irrigation solution is about 36 degrees, close to the body temperature is appropriate. Lavage time is generally chosen after breakfast or dinner 30-60 minutes. It is best to irrigate every day or every other day at a certain time and with a certain amount of water, which is conducive to promoting the formation of bowel regularity. If there is abdominal pain and other discomforts during the irrigation process, the irrigation must be temporarily stopped (do not take off the conical irrigation head and still press it at the opening of the stoma, and at the same time take slow deep breaths, wait for the disappearance of abdominal pain and then continue to irrigate, and do not force the pressure to irrigate, so as to prevent accidents from occurring. If there is nausea or vomiting, the irrigation should be suspended. In winter and in the north, stoma patients should pay attention to the warmth of the abdomen when irrigating. Avoid getting cold and causing discomfort. After irrigation, please don’t forget to record the amount of water for next time’s reference, and also pay attention to the time of next bowel movement. For example, if there is a bowel movement 48 hours after irrigation, this indicates that irrigation should be performed every 48 hours. At the beginning of irrigation, patients with stoma are not yet proficient in the method, so in order to prevent the irrigation fluid from flowing out, it is still necessary to use an ostomy bag to collect the irrigation fluid that has not flowed out.
