Allergic purpura is a common disease in pediatrics, especially in our area, where hundreds of such patients are seen and treated every year. In the process of dealing with these children, I have discovered many issues that I would love to talk to my parents and friends about. One of these critical and most important questions is: Where exactly should a child go when he or she unfortunately has allergic purpura, or is suspected of having allergic purpura? In clinical practice, in addition to treatment in pediatrics, we often see children who are also admitted to adult dermatology, hematology, nephrology, gastroenterology, and immuno-rheumatology. However, from the point of view of a medical professional, I would recommend treatment in pediatrics. The incidence of this disease is much lower in adults or older adults than in children, so pediatricians have a much better chance of treating this disease than clinicians in adult departments and have relatively more clinical experience; therefore, as long as the child is within 18 years of age, he or she should be treated in pediatrics. The disease is a small vasculitis caused by a systemic immune response, which can involve the skin, joints, kidneys and gastrointestinal tract, and even the pancreas, brain, lungs, etc. The disease is not a hematological disease, so it is not appropriate to stay in the hematology department; the disease is not only a skin lesion, but a systemic disease, so it is obviously not appropriate to stay in the dermatology department; The disease is easy to invade the kidney, but if you live in the nephrology department, the nephrologist can’t deal with the gastrointestinal lesions and is not familiar with allergic purpura, so the disease is also not suitable to live in the nephrology department; gastrointestinal symptoms are only part of the clinical manifestations of allergic purpura, living in the gastroenterology department can only deal with abdominal pain, blood in the stool and other symptoms, it can be seen that living in the gastroenterology department also can’t get comprehensive treatment; immuno-rheumatology department can treat allergic purpura However, it is not convenient to deal with kidney damage. Therefore: parents and friends are advised to send their children with allergic purpura to a larger hospital pediatric clinic! The reasons are as follows: 1, the first symptoms of allergic purpura manifest in a variety of ways, easy to misdiagnose. Especially, there are some patients with allergic purpura who can have no typical rash in the early stage, but with abdominal pain as the main manifestation, often in some inexperienced hospitals as appendicitis did surgery. When the abdominal pain is still not relieved after the surgery is done and the typical purpura gradually appears, only then do you think about the allergic purpura. 2, some inexperienced doctors can not correctly grasp the indications and course of hormone use. The use of hormones too much or the course of treatment is too long will produce unnecessary side effects, and no that is reasonable use of hormones, and will delay the disease, leading to serious complications, causing consequences that could have been avoided. 3, the skin damage of allergic purpura is not terrible, generally can be very good control. But if the treatment is not appropriate, or some comprehensive measures are not appropriate, it may lead to recurrence of the rash. Often and parents and friends say this: “a rash, bite a kidney”, it can be seen that repeated rashes will lead to an increased chance of kidney damage. 4, because a few doctors do not know enough about the degree of allergic purpura kidney damage, did not take measures to prevent kidney damage, often after the purpura subsides and then appear hematuria, proteinuria, if not treated in time, it will delay the disease. 5, if there is kidney injury, the course of the disease can be very long, up to 1-3 years, and even the child’s life may have to pay attention to test urinary routine and kidney function. In the end, how long to cure, and whether the recurrence of new kidney damage is very close, parents and friends must pay enough attention to! 6, often see some originally very common allergic purpura, because the parents have some fluke, or due to economic reasons, or due to affairs, or due to traffic inconvenience, first in the nearby medical consultation, treatment effect is not good and then transferred to our hospital, both increased the economic burden, and delay the best treatment period. Families regret and we deeply regret. However, there are few children with allergic purpura who have a mild condition and a clear self-limiting nature and will be cured after simple treatment. For such children, the dynamics of the urine routine should be closely monitored for at least 1-3 months. In conclusion, it is highly recommended that parents and friends must pay enough attention to allergic purpura.
