New Frontier in Tumor Treatment – Thermotherapy

Overview Hyperthermia is a physical method to treat tumor by heating, using the thermal effect generated by the precipitation of physical energy in the tumor tissue to heat the tissue to a temperature that can kill the tumor cells, so as to destroy the tumor cells without damaging (or less affecting) the normal tissue. Heat therapy not only has a direct killing effect on tumor cells, but also can enhance the efficacy of chemotherapy and radiotherapy, improve the immunity of the body and inhibit the metastasis of tumor, which becomes another effective treatment method after surgery, radiotherapy and chemotherapy. Heat therapy does not cause adverse effects such as bone marrow suppression and hair loss. This is the unique feature that distinguishes it from chemotherapy and radiotherapy, therefore, it is also called green therapy. According to the treatment area, tumor heat therapy is mainly divided into systemic heat therapy and local heat therapy. Systemic thermotherapy is seldom used in clinical practice because of its complicated procedures, many complications and large damage to patients. Local heat therapy is also divided into microwave heat therapy, ultrasonic heat therapy and radiofrequency heat therapy according to its principle. At present, the deep tumor phased-array thermal therapy system (BSD2000, USA), high-energy ultrasonic focused tumor treatment system (HIFUNIT9000, China) and tumor radiofrequency therapy machine (RITA1500X, USA) owned by the Department of Oncology of Beijing Hospital of Traditional Chinese Medicine all belong to local thermal therapy, and the roles and characteristics of various thermal therapy means are introduced below. 1.Introduction The BSD2000 thermal therapy system is a deep tumor thermal therapy machine with a perfect three-dimensional planning system, which establishes a three-dimensional treatment plan for human body by collecting data from CT or MRI of patients. The system uses phased array focusing technology, and the energy can be adjusted by the phase and amplitude of each channel on the heat therapy machine to form a heat field suitable for the shape and size of tumor, which can well heat the tumor parts in the body precisely and reach the temperature of 41.5℃~43.5℃ required for treatment. During the treatment, the patient’s internal temperature is monitored in real time by the advanced temperature measurement system attached to the treatment, thus it will not harm the surrounding normal tissues. 2.Treatment characteristics (1) Non-invasive treatment (no open surgery) (2) No postoperative infection and no intraoperative metastasis. Indications: (1) Tumor diseases Deep tumors in the body, such as lung cancer, stomach cancer, liver cancer, intestinal cancer, breast cancer, malignant lymphoma, pancreatic cancer, bladder cancer, metastatic cancer in the chest, abdomen, pelvis and extremities, etc. Body surface tumors include skin cancer, soft tissue sarcoma, bone metastases, malignant melanoma and head and neck tumors, etc. (2) Benign diseases Commonly used in gynecological, surgical and dermatological diseases: such as mastitis, pelvic inflammatory disease, adnexitis; acute and chronic inflammation of soft tissues, arthritis, prostatitis; herpes zoster, skin eczema, chronic dermatitis, intractable skin ulcers, etc. Especially for pelvic inflammatory disease and chronic skin ulcers that do not heal repeatedly, the combination of heat therapy on the basis of systemic treatment can often achieve satisfactory results. High Intersity Focused Ultrasound (HIFU) 1.Introduction High Intersity Focused Ultrasound (HIFU), also known as HIFU, ultrasound knife, is the use of ultrasound penetration depth, strong directivity, good focusing performance, the majority of low-energy ultrasound gathered, the use of water-mediated coupling effect, deep inside the human tumor, instantly. The ultrasonic knife is used to penetrate deeply into the interior of human tumor, instantly heating up the target tissues to 60-90℃, producing high temperature effect, mechanical effect and cavitation effect. It causes the tumor tissues in the target area to produce qualitative changes mainly coagulative necrosis and lose the ability of proliferation, infiltration and metastasis, but has little effect on the normal tissues outside the target area. At present, HIFU technology in China is the leading level in the world. 2.Treatment characteristics (1) Treatment under real-time monitoring of ultrasound, “conformal treatment” can be performed according to the size and shape of the tumor to ensure treatment accuracy and safety. (2) The treatment process is generally painless (or only slightly painful in the treatment area but without anesthesia). There are no complications such as perforation and bleeding. (3) No radioactive contamination. (4) Synergistic effect with radiotherapy combined with grams. 3.Indications It is mainly applicable to solid tumors in human pelvic and abdominal cavities that have complete ultrasound access and can be observed in its entirety by using the built-in ultrasound probe. (1) pelvic and abdominal malignant tumors: especially patients with inoperable advanced tumors, including: pancreatic cancer, liver cancer (left lobe), pelvic or abdominal recurrent and metastatic tumors (including lymph node metastatic cancer), prostate cancer, soft tissue sarcoma, uterine cancer, bladder cancer, kidney cancer, etc. (2) Benign diseases: uterine fibroids, uterine adenomyosis, prostate hyperplasia, soft tissue benign tumors, etc. Sub Malignant tumor radiofrequency ablation (RFA) 1. Introduction Radiofrequency ablation (RFA) is to percutaneously puncture the radiofrequency electrode needle to the center of the tumor lesion in the body under the guidance of CT or B ultrasound, using alternating current to make the ions in the tumor tissue oscillate and generate heat by friction, thus increasing the temperature of the tumor area to about 100. oC, which eventually leads to coagulative necrosis of the tumor tissue and achieves the treatment purpose. The necrotic tumor tissue will be gradually absorbed and dissipated by human body. (1) It can precisely destroy tumor tissues; the efficacy of radiofrequency ablation for small tumors is the same as that of surgical resection. (2) It is a minimally invasive treatment, which is less painful and can be tolerated by patients in general.
