Abstract: A 61-year-old female patient complained of sudden weakness with dizziness and tinnitus more than 10 days ago, which was never relieved after rest, so she came to the hospital for examination and treatment. Combining the patient’s clinical symptoms, physical signs and pathological examination results, colon cancer could be diagnosed, and the patient was immediately given surgery and medication, and the patient’s condition was controlled.
Basic information】Female, 61 years old
Disease Type】Colon cancer
Hospital】The First Affiliated Hospital of Xi’an Jiaotong University
Date of consultation】February 2022
Treatment plan】Surgical treatment (right hemicolectomy) + intravenous drugs (irinotecan hydrochloride injection, ceftriaxone sodium for injection)
Treatment period】In-hospital treatment for 24 days, monthly review by doctor
Treatment effect】The disease has been controlled and the symptoms of diarrhea, weakness and bloody stools have disappeared.
I. Initial consultation
When we first saw the patient, he reported that he had a history of hypertension and type 2 diabetes and was taking the appropriate medication daily. More than 10 days ago, he suddenly felt weak, accompanied by dizziness, tinnitus, panic and chest tightness after activity, epigastric pain and discomfort, occasional nausea, no vomiting, and poor eating, and was admitted to the hospital to determine the cause of his discomfort. During physical examination, the patient was found to have a mass in the abdomen and a positive fecal occult blood test. Colonoscopy showed that there were many small grayish-red tissues with soft texture in the liver area of the colon, which was suspected to be colon cancer. The patient’s clinical manifestations, physical signs and colonoscopy results were combined, and he was admitted with colon cancer.
II. Treatment process
After the patient was admitted to the hospital, the pathological examination showed that adenocarcinoma (of the colon and liver), i.e. colon cancer. After comprehensive consideration, we first communicated with the patient and decided to perform right hemicolectomy as soon as possible, and the patient actively cooperated with the treatment after hearing that. Before surgery, the patient was given general anesthesia, and the surgical area was disinfected. A linear incision was made in the right side of the patient’s abdomen, and the right hemicolectomy, the right end of the transverse colon, and the right hemicolectomy were removed. After the patient was awake, he was safely returned to the ward. After the operation, the patient was given anti-infection treatment with ceftriaxone sodium for injection and chemotherapy to kill cancer cells and control the spread of cancer cells.
III. Treatment effect
After 24 days of in-hospital treatment, the patient’s condition was under control and the symptoms of diarrhea, weakness and bloody stools disappeared. His mental status, diet, sleep and urine and stool were normal, and he did not have symptoms such as nausea, vomiting, chest tightness and chest pain. On physical examination, the patient’s abdomen was flat, with soft abdominal muscles and no pressure pain, and the surgical incision was healing well without redness, swelling and exudation.
IV. Precautions
When the patient’s condition improved and was happily discharged, I think I felt very happy in my heart, but at the time of discharge I still want to advise the patient to pay attention to the following points.
1. Although the patient was discharged from the hospital, he was still relatively weak, so I suggested that he should take rest after going home, not to exert himself too much, and to keep his mood relaxed.
2, diet to ensure a balanced daily dietary intake, appropriate intake of high-fiber foods, such as cucumbers, apples, bananas, celery, etc., appropriate daily eating can be conducive to stomach peristalsis, promote food digestion, and thus prevent constipation, and to develop the habit of regular bowel movements, which can help the condition to improve.
3, I also repeatedly instructed the patient to visit the doctor on a monthly basis for review, so as to monitor changes in the condition at any time.
V. Personal insight
Colon cancer is a disease that can be treated promptly in the early stage and with good life care, the prognosis of the disease is better and can improve the quality of life of the patient and inhibit the spread of cancer cells to other parts of the body, which can endanger their lives. Just like the 61-year-old female patient in this case, she chose to seek medical treatment when she found abnormal symptoms instead of blindly taking medication on her own, which I also advocate. In addition, I also recommend that you pay more attention to your health and seek medical checkups on time.
