Ejaculation, also known as inability to ejaculate, refers to the absence of semen flow during intercourse and the absence of orgasm; it is a manifestation of ejaculation disorder and is more common among men with sexual dysfunction. Ejaculation can cause infertility in men, and a large proportion of such patients come to the clinic with the aim of treating infertility. Most of the common clinical cases of non-ejaculation are psychogenic, and a very small percentage are caused by organic factors. Modern medicine believes that people with advanced neurological activity, their reproductive organs to play a normal ejaculatory function, in addition to sound neurological —- endocrine control and sexual organs, the two basic conditions, the cerebral cortex of the sexual conditioned reflexes play a leading role; secondly, the human spirit, psychological state, health status, knowledge of sex, living environment and emotional communication between husband and wife, will also directly and indirectly affect the ejaculatory function. The physiology of ejaculation is a complex reflex process coordinated through the central nervous system, sympathetic and parasympathetic nerves, sex hormones and the normal reproductive organs multi-system. The physiology of ejaculation can simply be composed of three parts, namely, the discharge of semen into the posterior urethra; the closure of the bladder neck; and the ejection of semen from the posterior urethra toward the external orifice, i.e., ejaculation. These three parts are controlled by different nerves, and abnormalities in any one of them can affect ejaculation. In Chinese medicine, there are many different types of ejaculation, such as liver stagnation and fire, blood stasis, yin deficiency and fire, kidney yang deficiency and heart and spleen deficiency. According to the cases I have treated for many years, most of them are due to lack of sexual knowledge, emotional and mental discomfort, thoughts and fears, resulting in imbalance of qi, stagnation of liver channels, poor qi and blood flow, and loss of spermatic ventricles and channels. In addition, various causes of qi stagnation, dampness, dampness and heat, poisonous evil or trauma can lead to obstruction of the orifices of the ligaments and veins, depression of the sperm chamber and inaccessibility, thus leading to non-ejaculation. Therefore, stagnation of the liver and veins and obstruction of the sperm ventricles are the main pathogenesis of ejaculation. As this disease is mainly due to mental and emotional conditions, the general pathogenesis is that various causes lead to depression of the liver meridians and veins, stagnation and obstruction of the mental orifices, so that the semen cannot be discharged and the ejaculation occurs. The main method of Chinese medicine for treating non-ejaculation is to open up the liver channels, sperm orifices and water orifices. Most of the clinical prescriptions are based on the combination of Si Wei San and Ma Huang Hsiang Gan Cao Tang with additional flavor. The commonly used drugs are Chai Hu, Citrus Aurantium, Bai Shao, Wang Bu Liu Xing, Xiang Shen, Lu Lu Tong, Acute, etc., as well as Ephedra and Hsiang, which pass through the sperm orifice, and Shi Calamus and Che Qian Zi, which pass through the water orifice. Although pungent, warm and cold dispersing medicine, but pungent has a good role in opening the orifices. Ephedra can ventilate the essence and open the orifices, which is a good product for opening the orifices and draining the menstruation. The main components of ephedra are ephedrine and pseudoephedrine. The pharmacological effects of ephedra are mainly due to the pharmacological effects of ephedrine, which improves the patient’s mental tension by stimulating the cerebral cortex and cortical center, increases the tone of the bladder sphincter by slowing down the peristalsis of the gastrointestinal muscles, improves the closing ability of the bladder neck opening; contracts the smooth muscle of the vas deferens, thus indirectly promoting the function of seminal fluid discharge. In addition, the pseudoephedrine contained in ephedra has a good water-releasing effect. Ephedra enters the lung and bladder meridians and has the function of promoting lung qi and regulating the water channels. It can synergistically promote the water channels with psyllium to relieve the tension of the bladder muscle wall, thus improving the tension of the bladder sphincter and promoting the contraction of the bladder neck opening to perform normal ejaculatory functions. For the treatment of non-ejaculation, not only from medication, but also psychological and behavioral induction synchronous treatment is needed to enhance the stimulation response of the penis to reach the onset of orgasm and produce ejaculation.
