Persimmons are delicious and difficult to digest. Recently, two patients were admitted with persimmon and intestinal persimmon stones, one of them almost died, which brought a profound lesson and pain to both the patient and the family. So today we will talk about persimmon stone disease. Let’s first look at the abdominal CT and gastroscopy findings. Large round persimmon stones are seen in the stomach Persimmon stones are stuck in the sinus and block the exit of the stomach Large brown persimmon stones are seen on gastroscopy Can eating hawthorn also cause persimmon stones? Many people know that eating persimmons, persimmon cakes and black dates (unique to the north) can cause persimmon stones, but very few people know that eating hawthorn can also cause persimmon stones. Why can persimmon stones be caused by eating persimmons and hawthorn? Persimmon and hawthorn contain a large amount of tannic acid (tannic acid), and when they encounter acidic substances, especially stomach acid, the tannic acid denatures the proteins in the food under the action of gastric digestive enzymes and combines the food and residues into a mass, forming persimmon stones. The longer it is in the stomach, the stronger the masses form, which we call persimmonite, or persimmonolithiasis. What are the dangers of persimmonithiasis? If you eat few persimmons (cakes) or hawthorn, the persimmon stones formed are small and can usually be excreted through the stomach and intestines and the patient can have no symptoms. If you eat a large amount of persimmons or a large number of persimmons (cakes) continuously, some patients will form very large persimmon stones, which cannot be expelled from the stomach and will block the gastric pylorus, causing abdominal pain, nausea and vomiting. Even if some persimmon stones are barely discharged from the gastric pylorus, it is difficult to pass through the small intestine exit (ileocecal), which will cause intestinal obstruction, especially in elderly people with low bowel function and constipation. Severe intestinal obstruction without timely consultation can lead to intestinal necrosis and rupture, causing diffuse peritonitis and life-threatening. We admitted an 80-year-old man with persistent small bowel obstruction and intestinal rupture a week ago, and he gradually recovered after resuscitation and was transferred from the intensive care unit to the general ward only 3 days ago. What are the clinical manifestations of persimmonolithiasis? 1.History of eating persimmon (cake), black dates or hawthorn, often in large quantities. 2.Supra-abdominal fullness and distension, decreased appetite, nausea and vomiting, and more severe symptoms after eating water, 1 or 2 days after eating. 3.Some patients have pain in the whole abdomen, nausea and vomiting, and stopping defecation and exhaustion. 4.Some patients with severe abdominal pain and vomiting aggravated, even shock, life-threatening. How to deal with? 1, patients with the above history and symptoms promptly to the hospital gastrointestinal surgery, if necessary, emergency. 2, combined with the medical history, clinical manifestations, after gastroscopy, imaging or CT and other examinations most can confirm the diagnosis. 3. Once the diagnosis is confirmed, the stone can be removed under gastroscopy, and those who have difficulty in removing the stone need surgery. 4.If intestinal obstruction is combined, surgery is needed as soon as possible after conservative treatment has no effect. 5. Depending on the patient’s age, the length of the disease, the criticality of the disease and the local medical level, lumpectomy or open surgery can be performed. How to prevent persimmon stone disease? 1, eat less persimmon, do not eat a lot of persimmon, hawthorn, etc. in a short period of time, especially when fasting. 2, early persimmon stones within two days, if there is no obvious obstruction in the stomach and intestines, you can take oral sodium bicarbonate tablets to alkalize the gastric juice, and individual persimmon stones can dissipate. 3, after eating persimmon can be oral cola has a preventive effect, but there is a lack of research data. 4.Early persimmon stones within 2 days can be taken orally with cola, which has the auxiliary effect of dissolving and dissipating, but it can be tried only if the doctor determines that there is no gastrointestinal obstruction, and is not recommended for routine use.
