Abstract: When 30-year-old Ms. Wang visited our hospital, she complained of frequent unexplained nasal bleeding, accompanied by dysphagia hoarseness and nasal congestion. After palpation, EBV serological examination, nasopharyngoscopy and other examinations, Ms. Wang was initially diagnosed with nasopharyngeal carcinoma, and after Ms. Wang’s informed consent, her nasal bleeding symptoms improved significantly and her disease basically recovered after 7 weeks of standardized treatment.
Basic information】Female, 30 years old
Type of disease】Nasopharyngeal carcinoma
Hospital】Shanghai Changhai Hospital
Date of consultation】May 2019
Treatment plan】Radiotherapy treatment (high-energy radiotherapy) + chemotherapy treatment (paclitaxel injection) + targeted drug treatment (pertuzumab injection)
Treatment period】7 weeks of drug treatment, regular review
Treatment effect] The physical discomfort was gradually relieved, and the test results returned to normal.
I. Initial consultation
Ms. Wang, 30 years old, came to our hospital complaining of frequent nasal bleeding and blood in her nasal discharge recently.
Through palpation, I found that Ms. Wang had enlarged lymph nodes in her neck and found a swelling in her neck. The nasopharyngoscopy revealed a nasopharyngeal mass, and the MRI of the neck confirmed the diagnosis of mid-stage malignant tumor, and she was admitted to our hospital for treatment of nasopharyngeal cancer.
II. Treatment process
After the diagnosis of nasopharyngeal cancer, because there were more blood clots and secretions in Ms. Wang’s nasal cavity, with her informed consent, firstly, 0.9% sodium chloride injection was used to clean up Ms. Wang’s blood and mucus, and radiation therapy such as high-energy radiation was used to destroy malignant tumor cells, while paclitaxel injection was given intravenously to inhibit the continuous growth of malignant tumor cells, and targeted therapy drug pertuzumab injection was given to block the blood vessels that nourish tumor cells. In addition, we will give the targeted therapy drug pertuzumab injection to block the patency of blood vessels that nourish the tumor cells, so as to promote the necrosis of malignant tumor cells and avoid the continued increase of tumor size.
III. Treatment effect
After 7 weeks of treatment by radiation therapy and using nitrozumab injection, Ms. Wang complained that the symptoms of nasal bleeding, nasal blockage on both sides, tinnitus and hoarseness disappeared, and she also felt that her sense of hearing, vision and smell had recovered, and her tongue had improved in flexibility and she could swallow food normally. I found that the swelling in Ms. Wang’s neck disappeared by palpation. The EBV serological examination was performed again and it was clear that the EBV level returned to normal. Using nasopharyngoscopy, I found that the swelling in the corresponding area disappeared, and through magnetic resonance examination of the neck, I found that the malignant tumor disappeared, so Ms. Wang was discharged from the hospital.

IV. Notes
We are glad that Ms. Wang’s condition was effectively controlled. We need to remind Ms. Wang that she can usually take more food that is rich in high quality protein and other nutrients, light and easy to digest, such as hot milk, egg custard, etc., which can help strengthen her body; in addition, pay attention to proper physical exercise, such as jogging, aerobics, yoga, etc., which can help strengthen one’s body, and at the same time, avoid inhaling harmful gases, and avoid traveling in bad weather such as foggy weather to avoid causing throat problems. In addition, it is important to avoid inhalation of harmful gases and travel in bad weather such as haze to avoid throat discomfort.
V. Personal insight
Nasopharyngeal cancer is a malignant neoplastic disease, which is mainly caused by EBV multiplying and infecting the nasal cavity and throat. Frequent consumption of food containing carcinogenic substances, inhalation of harmful gases, family history of nasopharyngeal cancer and weak health are all causes of nasopharyngeal cancer, so Ms. Wang should pay attention to avoid it as much as possible in her daily life. In addition, as nasopharyngeal cancer progresses, it will be more and more difficult to treat. Therefore, it is recommended to seek medical treatment as soon as possible after the appearance of discomfort to achieve early detection and early treatment.
