The decision to check for painful piles depends on the patient’s perianal and rectal environment. If the patient’s piles are small, unbroken and uninfected, with a good perianal environment and no other lesions in the rectum, the test is generally not painful and will only produce minor discomfort. If the patient has complications such as anal fissures or fistulas, there will be some pain during the rectal examination.
