Initial consultation Patient Xiaolin is a 17-year-old female high school student who came to our hospital because of hair loss on the top of her head due to her fondness for hair pulling. A simple examination of the patient revealed a large area of hair loss on the top of her head, and she was told that she had a habit of pulling out her hair, and that she often had a strong urge to pull out her hair when she encountered difficulties in her studies or when she was in an irritable mood. Initial suspicion of hair pulling fetish, so the patient for further questioning, the patient said that due to the fear of exam results are not satisfactory and high pressure, there has been a blurred vision and other conditions, usually more prone to sweating, so the skin on the face is always more red. Hair microscopy was arranged for the patient, and the results did not show any obvious abnormality. Taking into account the above symptoms and the patient’s psychological condition, the patient was diagnosed with hair pulling fetish. Considering that the patient’s hair-pulling fetish was caused by excessive psychological pressure, I provided psychological counseling to the patient, telling her that academic performance was not the whole of her life, and that if she felt difficulty in studying, she could seek help from her good friends, teachers or parents. At the same time, I also told the parents that they could stop their children from pulling out their hair when they made a move to do so. In addition, considering that the patient’s hair pulling phenomenon is serious and the hair loss is relatively large, so I gave the patient haloperidol tablets to help the patient control his behavior, and at the same time, with the addition of Qiju dihuangwan and electro-acupuncture treatment, in order to promote the growth of hair. After medication, psychological counseling and electroacupuncture treatment, the number of hair pulling of the patient was significantly reduced. After 6 weeks of medication, new hairs could be seen on the top of the patient’s head, and the symptoms of sweating and blurred vision were gradually reduced. 10 weeks after medication, the symptoms of sweating and blurred vision disappeared, and the hair on the top of the patient’s head was increased. After 2 weeks of stopping the drug, the patient had thick hair on the top of the head, and reported that the psychological pressure was reduced, and he could control his behavior well, and there was no hair loss or always pulling out the hair after stopping the drug. After a series of medication, psychological and electroacupuncture treatments, the patient’s symptoms of hair pulling have been significantly improved, and new hairs are gradually growing on the top of her head. As the patient is still young and in an important stage of learning and growth and development, the patient and his parents should pay attention to the following situations in their daily life: 1. The patient should maintain emotional stability in his daily life, and seek for appropriate ways of recreation to relax and reduce the psychological pressure, and the parents should also relax their demands on the patient appropriately, and should not only ask for achievements. If the patient has something on his mind, it is recommended to communicate with his parents, teachers and classmates in a timely manner in order to relieve his bad mood; 2, daily life to treat the area of the newborn hair should be carefully cared for, you can properly massage the scalp to stimulate the growth of hair follicles, do not believe in the network of prescriptions, the casual use of drugs, in order to avoid stimulation of the scalp, damage to the hair follicles, increasing the difficulty of the treatment; 3, the daily diet should be noted that light, do not eat too much sweet food, greasy food. The daily diet should be light, do not eat too much sweets, greasy food, so as not to lead to excessive oil secretion, which may also affect the growth of hair as well as the treatment of the situation. V. Personal perception Teenagers daily learning pressure is relatively large, the burden of schoolwork is heavy, some people may be difficult to correctly resolve their own pressure, but instead of pulling out hair and other behaviors, feel less hair as if the pressure can be less. However, this is actually an incorrect approach, which may lead to the aggravation of psychological problems, hair pulling behavior will intensify, and may even cause low self-esteem to the patient because of the lack of hair, which belongs to a kind of vicious circle. Therefore, if there is a situation like the patient in this case, it is recommended to consult a professional doctor in time to formulate an appropriate treatment method, focusing on the psychological aspects of the treatment.
