Does subungual herniation of the cerebellum always require surgery?

Incidental imaging findings with asymptomatic or mild symptoms can be followed up with regular outpatient review and continuous imaging, but early surgical treatment is required if symptoms progress or MRI confirms progression of spinal cavity.
