Life and death exist at the same time, accompanying each other and constituting the whole process of human life. With the development and progress of society and the improvement of people’s quality of life, the pursuit of the value of life and the improvement of the quality of survival have gradually become the new choice of people. Especially for patients with advanced cancer, how to face life and death correctly has become a difficult problem in front of them. According to statistics, there are about 2 million new cancer cases and 1.4 million cancer deaths in China every year. 50%-80% of cancer patients have pain, and up to 60%-90% in advanced stage. Pain not only restricts cancer patients’ activities, reduces appetite, and affects sleep, but also may produce anxiety, depression, paranoia, low self-esteem, and anxiety when the pain increases. How to make them pass away painlessly and with dignity, and how to make them and their family members face the reality and accept death with the help of medical staff, is especially important. A senior oncologist once said, “We may not be able to make the patients live a long life, but we can help them take a wide path of life. How to make advanced cancer patients face life and death correctly and how to help them walk their last journey in life are issues that medical professionals, family members and the whole society should be concerned about. We should choose suitable ways according to the actual situation, provide them with spiritual and psychological support as well as physical care, help them improve their quality of life to the greatest extent, and let them feel the warmth and true love on earth. For terminal patients, although some active resuscitation measures, such as cardiopulmonary resuscitation, tracheotomy, tracheal intubation and defibrillation, can prolong the life of cancer patients to some extent, the quality of life is extremely poor. By doing so, it not only increases the patient’s suffering, but also obviously increases the burden of the family. Therefore, for them, a high quality of life is far more important than living when life is about to end, even in the last days or last hours of life. 1 Help patients to establish correct values and view of death Death is not terrible, death means not only the negation of life, but also the affirmation of life. As Sima Qian of Western Han Dynasty said, “There is a death in a person, which is heavier than a mountain or lighter than a feather”. In China, “eugenics” has been deeply rooted in people’s hearts, while “eugenics” is still somewhat unfamiliar. Nowadays, with the development and progress of society and the improvement of people’s quality of life, the pursuit of the value of life and the improvement of the quality of survival has gradually become a new choice for people. The modern “bio-psycho-social” medical model also highlights “people-oriented” and adheres to “patient-centered” humanized medical services. Especially for cancer patients, death should make them realize that their life is limited, so that they can cherish it more and live a healthier, more colorful and more meaningful life in the remaining time, so that they can face the moment of death frankly and have a good and happy life and a happy and peaceful death at the same time. 2 Taking palliative care as an important means to relieve suffering For a patient with advanced cancer, the purpose of treatment is no longer a cure, but how to improve the patient’s quality of life, how to prolong survival, in short, how to let them live longer and better. Therefore, for patients with progressive disease, tumor-related symptoms and complications gradually increase, and pain, dyspnea, anorexia, malignant fluid, nausea and vomiting, constipation, malignant intestinal obstruction, fatigue, sleep disorders, paranoia, etc. become common symptoms for patients who cannot tolerate conventional anti-tumor treatments such as radiotherapy, chemotherapy and surgery, and palliative care should be given for the tumor and the complications caused, including: palliative surgery, palliative 3 Active attention to psychotherapy Psychotherapy should not be neglected. The psychological characteristics of advanced cancer patients mainly include anxiety, loneliness, fear and despair, and finally recognition and acceptance. Helping cancer patients to correctly understand and appreciate death and reduce suffering seems to be crucial. Appropriate help should be provided at the right time according to their psychological stages and individual differences. During the denial stage, we should listen to them patiently in an amiable manner, understand and help them, and make them feel cared and taken care of; during the anger stage, we should allow them to vent their emotions; we should try to be patient, understanding and sympathetic to unwarranted rebuke and anger, and avoid arguing with them; during the agreement stage, we should try our best to reduce their pain; during the depression stage, we should give them comfort and strengthen their companionship, so that they can vent their depressed feelings In the acceptance period, we should respect them, not force them to talk, and strengthen basic care, so that they can feel the profound charm of “death is like the quiet beauty of autumn leaves” in the face of inescapable death.4 Comprehensive attention to end-of-life care Hospice care is a form of medical treatment and care for patients with limited time to live (6 months or less), either in hospital or at home, to reduce symptoms and slow down the progression of the disease. The purpose of hospice care is to reduce the psychological burden of the terminally ill patient, so that the patient can pass away peacefully, quietly, painlessly, comfortably and with dignity in a warm and loving atmosphere in the last years of life.
