Preparing for adenomyosis-conserving surgery? Take this pre-op guide

  The other day a patient who was preparing for surgery suddenly said during the preoperative conversation that he was still taking antihypertensive medication (because his blood pressure would increase from time to time, and since the preoperative tests were normal, he didn’t mention it again). The surgery was postponed, otherwise it would have been really dangerous! So today, after the surgery, I decided to write about the things I need to pay attention to before the uterus preservation surgery, hoping that the patients will pay attention to it, and I hope our doctors will pay more attention to it. Because once something happens, it may be irreversible.  First of all, it is important for patients to inform your doctor in charge of their past medical history, allergy history and medications in detail during the hospitalization. This is the only way to get a clear picture of your condition and help you overcome the disease. We also need to do these tasks before surgery and write them out for adenomyosis patients who will come for surgery to prepare in advance.  Pre-surgery examination Before your surgery, your doctor will carefully examine your whole body, paying special attention to blood pressure, heart, liver and kidneys. Any complications affecting the surgery will be treated first, followed by surgery. Local examination includes: visual examination of abdomen, vulva and vagina, vaginal duplex examination to understand the pelvic organs, and triple examination if necessary.  Pre-operative clinical tests 1. Blood tests (routine blood, blood type, coagulation time, liver and kidney function, glycolipids, infectious diseases, tumor markers, endocrine and thyroid function tests if necessary).  2.Urinary routine, vaginal secretions, cervical cytology (TCT), etc.  3. Functional examinations: frontal and lateral chest examination, electrocardiogram, ultrasound (uterine adnexa, liver, gallbladder, spleen, pancreas and kidney, breast, thyroid) and other examinations. Special patients will also need additional special tests, such as CT, MRI, cardiac function tests, etc.  Preparation of mind The doctor in charge will brief you and your family about your condition, explain the arrangement of the surgery and what may happen during the operation. If you have any concerns, please communicate with the supervising surgeon in a timely manner. If necessary, the night before the surgery, we give you appropriate sedation to ensure you have sufficient sleep.  Physical preparation 1. Adaptive exercise: Long-term smokers should quit smoking immediately to prevent postoperative coughing and lung infection, which may affect wound healing. For those whose postoperative condition requires prolonged bed rest, the nurse will instruct you to practice bed rest and bowel movements before surgery.  2. Diet management: For general surgery, it is advisable to choose light and easily digestible food at noon on the day before surgery. A semi-liquid diet, such as porridge, should be the main food before 8:00 pm. Fasting water in the morning of the day of surgery. You need to fast for 12 hours and 8 hours before surgery. Please follow the doctor’s instructions strictly to prevent anesthesia accidents and to ensure the smooth operation so that the gastrointestinal function can be restored as soon as possible after surgery.  3. Intestinal preparation: In order to prevent feces from contaminating the surgical field (the scope of the surgical site prepared before surgery) and to prevent intestinal damage during surgery, we need to prepare your intestines. You will start to take 3 boxes of polyethylene glycol electrolyte dispersion orally with water in the afternoon before surgery, and get out of bed appropriately for about 1~2 hours, and you will have diarrhea reaction.  According to the need of surgery, the nurse will give you an enema, there may be slight abdominal distension during the enema, there will not be pain and obvious discomfort, please do not worry.  4.Skin preparation: We will prepare your skin (remove hair from the surgical area) before the surgery to reduce post-operative incision infection. After the skin preparation, please take a shower, change your clothes, trim your finger (toe) nails and pay attention to cleaning your umbilicus.  5. Vaginal preparation and catheterization: Depending on the need, you will be given adequate vaginal disinfection after admission to the hospital until the surgery, and catheterization and preoperative medication will be required before the surgery.  6.Blood allocation: Before the gynecological surgery, the nurse will collect blood from anemic patients for blood type check and cross-matching test to ensure the safety of the surgery, and the doctor will consult you about the blood transfusion and sign the blood transfusion consent form.  7.Other: Before the surgery, please change your hospital gown, do not wear any underwear, do not wear dentures and jewelry, and leave your valuables to your family for safekeeping. The nurse will check your vital signs. If you have high body temperature or abnormal blood pressure, the nurse will inform the doctor and stop the operation if appropriate.  Noon: Good digestible food, noodles, rice, steamed buns, and meat as little as possible.  Evening: porridge (millet porridge, rice porridge, oatmeal, lotus root powder, sesame paste, thin soup noodles). Fasting and abstaining from food and water for 8-12 hours before surgery.
