The methods to quickly reduce swelling in the feet of the elderly can be selected according to the cause of edema, usually by elevating the feet to relieve edema (except for cardiogenic edema), taking diuretics or drugs to improve microcirculation, including Diosmin and Myzolyn. At the same time, the primary cause of foot edema should be treated actively to avoid delaying the disease and causing adverse consequences. I. Cardiogenic edema: diuretics are drugs to control fluid retention in the treatment of heart failure. 1. tab diuretics: furosemide is a strong diuretic, which can drain sodium and potassium and rapidly relieve foot edema; 2. thiazide diuretics: such as hydrochlorothiazide, which can inhibit the reabsorption of sodium and potassium and is optional for patients with mild heart failure edema; 3. potassium-protective diuretics: the diuretic effect is weaker compared with the other two drugs, commonly used spironolactone, aminoglutethimide, etc., which are mostly used with the above drugs. It is often used in conjunction with the above two drugs and to prevent hypokalemia; 4, arginine pressor receptor antagonist: tolvaptan, which inhibits water reabsorption and has little effect on electrolytes. Second, hepatogenic edema: lower limb edema is mostly accompanied by peritoneal effusion, which can be treated with combined spironolactone and furosemide. If the diuretic effect is poor and accompanied by hypoproteinemia, it can be treated with intravenous albumin infusion as appropriate, while sodium and water intake should be restricted and treatment of primary diseases should be carried out to improve foot edema as much as possible. Third, endocrine edema: 1. Primary aldosteronism: oral spironolactone inhibits aldosterone, preserves sodium, water and potassium, which can effectively improve edema and correct hypokalemia. In addition, those who meet the indications for surgery should undergo surgery to remove the adenoma that secretes aldosterone, which has a better therapeutic effect on the disease; 2. Cushing’s syndrome: water and sodium retention caused by excessive secretion of adrenocorticotropic hormone, cephradine and methylphenidate can be used to reduce the synthesis and release of cortisol, thus alleviating edema. Surgical removal of adrenal adenoma or pituitary tumor is recommended to reduce the secretion of adrenocorticotropic hormone. IV. Lower limb venous thrombosis: anticoagulant drugs, such as warfarin or rivaroxaban, should be taken to treat thrombosis, improve venous reflux and reduce edema, and if necessary, lower vena cava filters should be placed to prevent pulmonary artery embolism. V. Nephrogenic edema: nephrotic syndrome, glomerulonephritis, renal failure, etc. may cause foot swelling, which needs to be treated for the primary nephropathy and can be treated symptomatically by combining diuresis, etc.
