What is minimally invasive surgery

First of all, I would like to talk about minimally invasive surgery, because it is the most common question I get asked every day, probably because I have been involved in minimally invasive techniques and surgery. But I think the main reason is the inconsistency in the understanding of minimally invasive surgery among “everyone”, which I mean more senior surgeons. The reason for this inconsistency is that there is no standardized standard for minimally invasive surgery at home and abroad. In the case of otologic surgery, microscopic surgery is generally considered to be minimally invasive, but with the rise of endoscopy in recent years, some otologists consider endoscopic surgery to be minimally invasive. As far as I know, there is a trend among otologists that older doctors pay less attention to endoscopic techniques, while younger doctors pay too much attention to endoscopy, and some middle-aged and younger specialists even equate endoscopic surgery with minimally invasive. This not only confuses patients’ understanding and acceptance of minimally invasive surgery, but also is detrimental to the development of minimally invasive surgery. So, what exactly is minimally invasive surgery? From my own understanding, “minimally invasive” is a philosophy, the soul of the surgeon, and should be used throughout the entire surgery. The surgeon must always keep in mind that the surgery should cause as little damage as possible to healthy tissue structures, that is, to minimize trauma and maximize functional protection based on the safe, accurate and effective removal of lesions. This can be said to be the true meaning of minimally invasive surgery. Therefore, whether it is a microscope, an intraocular lens, or any other device, it should be a surgical tool that serves the “minimally invasive” concept. The surgeon needs to apply these tools scientifically and rationally to serve the “minimally invasive” concept during the surgery. At this point, I think you can know my answer to the question of minimally invasive surgery. In a way, minimally invasive surgery is a surgery done by a surgeon with a minimally invasive philosophy and soul, using various tools and methods. Of course, exactly how each procedure is minimally invasive may vary from surgeon to surgeon. Next, I will take time to gradually share my minimally invasive approach in specific surgeries, hoping to be helpful to my patients and inspiring to my colleagues, and welcome you to exchange and learn from each other to make progress together.
