How is inflammation of the residual stomach treated?

  Residual gastritis is the most common disease after subtotal gastrectomy, with an incidence of 60% to 100% of gastrectomies [1], usually occurring from a few months to a few years after surgery, with the shortest being half a month after surgery and the longest being 20 years after surgery. There is no better treatment in Western medicine, and it often depends on re-operation. This disease belongs to the category of “bile disease”, “gastric gangrene”, and “gastric pain” in Chinese medicine. The author combined dialectical treatment with disease identification treatment with satisfactory results, and the experience is reported as follows.  1, liver and gallbladder fire disturbing the stomach, it is appropriate to clear the liver and gallbladder bile is attached to the liver, depending on the drainage function of the liver to drain the bile, through the power of the downward flow of gas into the intestine, and not to reverse. If liver heat moves to the bile, or bile fire burns inside, and the drainage is too much, the bile will not follow the normal way but overflow, and after total gastrectomy, the pylorus with “directional valve”-like function is missing, so that the alkaline intestinal fluid rich in bile flows back into the stomach and damages the gastric ligament, causing gastric mucosa congestion, edema, and even erosion and bleeding, anastomosis inflammation or pseudo-polyps formation, and mucosa brittleness. The mucosa becomes brittle, resulting in burning pain in the upper abdomen, dry mouth, bitter mouth, noisy belching, disturbed nausea, and even vomiting of yellow-green bitter water. After vomiting, the stomach can be temporarily relieved. The Ling Shu said: “evil in the gallbladder, rebellious in the stomach, the gallbladder fluid drainage is bitter mouth, stomach gas rebellious vomiting bitter”. Treatment is to clear the liver and gallbladder, regulate the elevation, accompanied by detoxification, eliminate swelling, remove rot and create new. We can use Artemisia Scutellaria Clear Gallbladder Soup, Elevation San, and Si Wei San with Yunnan Baiyao plus or minus. If the main problem is fullness and bitterness with yellow tongue coating, we can use Xiao Shen Che Tang, Wu Hui Wan and Sheng Sheng San with addition and subtraction.  For example, Guan, male, 42 years old, was first diagnosed on May 20, 1994. Two years after a major gastrectomy for refractory ulcer in the gastric sinus, the patient had burning pain in the stomach and epigastrium, bitterness in the mouth, and even vomiting of yellow-green bitter water for six months. He also had noisy belching, dry mouth, insomnia, and dry stools, one line in 2-3 days. He was treated with Chinese and Western drugs for a long time, but his disease was increasing and his weight was decreasing. Gastroscopy and pathological examination suggested: severe superficial erosive residual gastritis, anastomitis with bile reflux. The tongue was dark red with purple lining the edges, thin yellow tongue coating, and stringent pulse with count. Western medicine diagnosis of gastritis remnant. TCM diagnosis: bile disease. It is caused by internal incandescence of liver and gallbladder fire, dysregulation of Qi flow, bile and gastric Qi reversal, and bile liquid burning the gastric complex. The treatment is to clear the liver and gallbladder, regulate the elevation, with detoxification and swelling, expel decay and create new. The formula is based on Artemisia Scutellaria Qing Bile Soup, Elevation San, Si Wei San and Yunnan Baiyao with addition and reduction. This formula is based on the following formula: 30g each of Radix Scutellariae Sinensis and Radix Panax notoginseng, 10g each of Scutellariae Sinensis, Radix Scutellariae Sinensis, Radix Cicadae Sinensis, Turmeric, Radix Bupleurum and Radix Paeoniae, 15g each of Citrus aurantium, 10g each of Rhubarb [later] and Glycyrrhiza glabra, and 1g each of Yunnan Baiyao [punch]. The above formula is reduced to 4g of rhubarb with decoction, and 15g of half-xia, and the bitterness of the mouth is removed, the yellow moss is dissolved, and the pain is basically eliminated, with occasional noises. The original formula was removed from Huanglian and added 24g of Chinese wolfberry powder, which was chewed in 3 doses on an empty stomach and discontinued at the end of 55 doses. One week later, the gastroscopy was repeated, the original erosion foci disappeared and no bile reflux was seen. He was treated with Chai Shao Liu Jun Zi Tang with Citrus Aurantium, Yunnan Baiyao and Lycium powder for 2 weeks, and no recurrence was seen in the follow-up period of 6 months.  2, Chong Qi upward rebellion, it is appropriate to converge Chong Qi downward rebellion residual gastritis is mostly seen in middle-aged and elderly people. The former sage has said: “over 40 years, Yin deficiency and half”, the lower deficiency on the Sheng more, in addition to see the stomach and epigastric fullness, burning pain, noisy belching, bitterness and distress, some also see the waist and knee weakness, dizziness and insomnia, irritability and sweating symptoms, when the stool is dry and constipated, it is easy to appear vomiting heart. If the kidney deficiency is not astringent, plus the liver Qi arbitrary, it is easy to move the impulsive Qi, up the offense of the stomach, the stomach and the loss of descending and rebellious, and the absence of the pylorus, alkaline intestinal fluids then followed into the stomach, burning the gastric ligament, turbid Yin congestion, so the stomach and cavity full, noisy burning pain, belching mouth bitter; kidney deficiency loss of nourishment, so the waist and knee pain; liver Qi arbitrary, it is easy to stimulate anger. Constipation obstructs the downward flow of gas, so it is easily aggravated by depression and anger or constipation. Deficient Yang is disturbed by turbid Yin, resulting in dizziness and insomnia, irritability and sweating. Treatment is to tonify the kidneys, calm the liver, calm the flush, lower the rebellion and harmonize the stomach, with detoxification and swelling, and protect the membrane to produce new. We can use Zhang Xichen’s Stomach-suppressing and Stomach-suppressing soup combined with Yunnan Baiyao to add and subtract, and then consolidate with kidney-supplementing and stomach-suppressing products after the impulse is calmed.  For example, Jiang, female, 49 years old, was first diagnosed on March 30, 1995. She has been suffering from gastric mucosal prolapse with gastric ulcer for 12 years and has been suffering from gastric and epigastric fullness, burning pain and noisiness with dry mouth for 8 naive months, with lumbar and knee weakness, tiredness, irritability and insomnia, menstrual disorders, and the fullness and burning pain worsens whenever she is depressed and constipated, and often causes nausea and poor appetite. The tongue is dark red, the coating is thin, and the pulse is thin and stringent. Gastroscopy ① superficial – atrophic residual gastritis with bile reflux; ② erosive anastomosis. Western medicine diagnosis: residual gastritis, Hp negative. Traditional Chinese medicine diagnosis: gastric histoplasm. Kidney deficiency and loss of Qi control, liver Qi with Qi upward to offend the stomach, bile overflowing the stomach, gastric Qi upward rebellion. The treatment is to tonify the kidney, calm the liver, calm the impulse, lower the rebellion and harmonize the stomach, with detoxification and swelling, expel decay and create new. The treatment is to tonify the kidneys, calm the liver, calm the flush, subdue the rebellion and harmonize the stomach. In addition, 18g of Chinese wolfberry powder was chewed 3 times a day on an empty stomach. After 10 doses, nausea and bitterness disappeared, fullness and burning pain were significantly reduced, and stools became loose and soft. After taking 45 doses, he was able to sleep peacefully at night and his spirit improved. Gastroscopy review: mild superficial residual gastritis, the anastomosis was red and white, no bile reflux sign, the original erosion has disappeared, change to six Junzi Tang with wolfberry, yam, Panax ginseng powder and other conditioning consolidation.  From the point of view of identification, the core of the pathogenesis of residual gastritis is the imbalance of qi elevation and elevation and bile-stomach qi rebellion, which are caused by liver and gallbladder fire upwardly disturbing the stomach and impulsive qi offending the stomach. The treatment of residual gastritis should not only clear the liver and gallbladder, but also refer to modern medical knowledge and use the following methods to improve the therapeutic effect.  ①Increase gastrointestinal power and control bile reflux: On the basis of dialectic, select and add herbs that can enhance gastrointestinal power, such as Citrus aurantium, Citrus aurantium shell, Chai Hu, Dai Ochre, Rhubarb, etc., and choose 2~3 flavors. It is important to keep the bowels open frequently. Sichiban and its constituent drugs Chai Hu and Chai Hu and Citrus aurantium combined decoction have good effects in enhancing gastric emptying and small intestine propulsion (2) It can promote the downward movement of stomach qi.  (2) Detoxification and decongestion, killing H. pylori: H. pylori is one of the most important causes of chronic gastritis, and the severity of chronic gastritis is positively correlated with the number of H. pylori infections. The presence of a large number of aerobic and anaerobic bacteria after subtotal gastrectomy, even in the gastric mucosa of moderate to severe biliary reflux gastritis, there is a high rate of H. pylori infection (3) Therefore, according to the results of modern research on Chinese medicine, the combination of several drugs with the ability to kill H. pylori, can improve the effectiveness and shorten the course of the disease. Such as Huanglian, Scutellaria, Dandelion, Panax ginseng powder, Gui Zhi, etc.. Some of them can clear heat and detoxify the body, while some of them can also resolve blood stasis and reduce swelling. Lycium powder used externally can detoxify and disperse nodules, reduce swelling and pain, lift pus and create muscle (4) Chewed raw on an empty stomach, it has good detoxification and swelling, and protects the gastric mucosa.  (3) Invigorate blood circulation and eliminate blood stasis, reduce swelling and protect membrane, promote regeneration and repair of residual gastric mucosa: turmeric, Panax ginseng powder peach kernel, safflower and Yunnan Baiyao can invigorate blood circulation and eliminate blood stasis and swelling, increase blood flow of residual gastric mucosa and promote wound healing; Yunnan Baiyao and raw wolfberry powder used together can promote regeneration and repair of residual gastric mucosa.
