Ephedra, fine pungent and epiphytic soup is from the treatise on typhoid, original article 301: “Shao Yin disease, the beginning of which, counter fever, sunken pulse, ephedra, fine pungent and epiphytic soup is the main.” In his lecture notes on typhoid, Professor Liu Du Zhou suggested that Ephedra, Hesperidin, and Eucommia Soup is a formula for Sun and Shao-Yin diseases in which the wind-cold evil in the surface is not resolved and Shao-Yin Yang has been deficient. In more than 10 years of clinical practice in neurology, the author found that Ephedra sinensis and ephedra soup with addition and subtraction was used to treat some acute onset peripheral neuropathies in neurology with good efficacy, and several cases are presented below. Su Qiaozhen, Department of Neurology, Guangdong Provincial Hospital of Traditional Chinese Medicine 1 Li Mou, female, 63 years old, clinic number: 20363136, was admitted to the hospital on February 10, 2012 with the main reason of “tinnitus in the right ear, double vision for 1 month and dizziness for 1 day”. He was admitted with the following symptoms: mental clarity, mental fatigue, dizziness, sometimes rotation of vision, slight distortion of the mouth and eyes, double vision, tinnitus in the right ear, no numbness and weakness of the limbs, dry mouth and bitterness without the desire to drink, poor appetite, poor sleep, and regulation of the stool, with a light red tongue, thin white fur, and a sunken string pulse. Examination: right abducent nerve, right facial nerve, right auditory nerve injury, right hypoglossal nerve deficit. Previous history of hypertension. On admission, cranial examination showed multiple ischemic infarct foci in the cerebral bridge, bilateral thalamus, bilateral basal ganglia, bilateral radial coronas, and bilateral hemiaxial centers, with no clear acute infarction. Blood was drawn for a significant increase in blood glucose. After admission, combined with the patient’s symptoms and signs and MR findings to rule out cerebrovascular accidents, the diagnosis was considered to be multiple cranial nerve injury due to diabetic peripheral neuropathy. The patient was treated with glucose lowering, vitamin B1 and B12 intramuscular injection for nerve nutrition, and Chinese herbal medicine to activate blood circulation and remove blood stasis, and then the dizziness improved, but the visual double vision and the distorted eyes and mouth basically did not improve. After discharge from the hospital, the patient was re-examined at the outpatient clinic from March 1. Considering that the patient was an elderly woman with an acute onset and the orifices of the liver and kidney were damaged at the same time, the patient was considered to be suffering from a deficiency of Yang Qi in Shaoyin and the evil had entered Shaoyin. The main prescription: ephedra 9 g, raw eucommia (first decoction), dry ginger, roasted licorice 10 g each, hosanna (after the next) 3 g. 7 doses, 1 dose per day, water decoction, in 2 doses. 1 week later the patient re-examined: mouth and eye obliquity significantly improved, the right eyelid can be closed, bilateral nasolabial fissure basic symmetry, the right eye can be abducted but still not in place. After taking the above 14 doses, the diplopia disappeared, and the orofacial distortion basically recovered, but the tinnitus remained. The patient was not seen again.2 Sudden deafness Li Mou, male, 54 years old, hospitalization number: 6032322, was admitted to the hospital on March 1, 2012 due to “sudden onset of left tinnitus with hearing loss for 4 days”. He was admitted to the hospital with the following symptoms: clear mind, mental clarity, tinnitus in the left ear, hearing loss in the left ear, sleepiness, and bowel movement. The tongue is dark, the coating is white and thick, and the pulse is sunken. Previous history of hyperuricemia. Usually he works hard. Hearing test: Bilateral tympanogram type “A”, bilateral stapedius reflex elicited, considering neurogenic deafness. Diagnosis: TCM: deafness (Yang deficiency and external sensation); Western medicine: sudden deafness. After admission, he was given herbal medicine to activate blood circulation and remove blood stasis, and Tongkou and Blood circulation Tang with addition and subtraction, but there was no significant improvement. on March 6, he was considered to have acute onset of the disease, the ear is the organ of the kidney, and combined with the pulse, it was caused by the evil entering Shaoyin. from March 6, he was given Ephedra with Rhizoma and Rhizoma Fine Sinensis Tang with addition and subtraction. Prescription: 15g each of Ephedra, Radix et Rhizoma (decoction first), Radix et Rhizoma, Radix et Rhizoma Ginger, Radix et Rhizoma Glycyrrhizae. 1 dose per day, decoction in water, divided into 2 doses. After the second dose, the patient felt that his hearing gradually recovered. The patient was discharged with no significant improvement in tinnitus. 3 Peripheral facial nerve palsy patient Zhong Mou, female, 77 years old. The patient was admitted to the hospital on October 6, 2011, with “sudden onset of right-sided distortion of the mouth and eyes and incomplete closure of the left eye for one day”. He had a history of hypertension, gastrectomy for gastric perforation and cerebral infarction, with no significant sequelae. He was admitted to the hospital with the following symptoms: mental clarity, mental fatigue, lack of color, skewing of the corners of the mouth to the right, incomplete closure of the left eye, loss of frontal lines on the left side, dizziness, chest tightness, palpitations, no weakness or numbness of the limbs, distention and discomfort of the stomach and epigastrium, general sleep, and regulation of the bowel movement, light fat tongue, slightly thick white fur, and sunken pulse. Physical examination: left-sided peripheral facial palsy. After admission, the patient was given a Chinese herbal medicine to activate blood circulation and resolve blood stasis, and a Chinese herbal soup with the addition and subtraction of Zhanzheng San, which was not effective, and was discharged on October 22, 2011. After discharge, the patient was seen again in the outpatient clinic, and the herbal medicine to dispel wind and promote circulation was still ineffective, the author had lost confidence and repeatedly told the patient, but the patient still insisted on seeing the clinic. Because of the patient’s stomach discomfort and chest tightness, I considered to start with the stomach discomfort and gave him Han Xia Di Xin Tang plus reduction, but the effect was still not good. On January 14, 2012, considering that the patient was old and had been ill for a long time, and that the tongue and pulse had not changed significantly from before, the disease should be deeper, in the Taiyin and Shaoyin meridians, he was given Hemp with Fine and Xiao Shen Che Tang plus and minus. Prescription: Ephedra 9 g, Radix Rehmanniae (first decoction), Radix Fructus Panax, Psidium Guajava, Psidium Guajava and Radix Ginger 15 g each, Radix Sinensis (later) 3 g, Rhizoma Huanglian 5 g. 1 dose daily with water decoction. On March 14, 2012, the prescription was changed to Hemp, Herba, and Hosanna Tang with addition and subtraction to directly dispel the evil of Shao-Yin. Prescription: Ephedra 9 g, Radix Rehmanniae (first decoction) 15 g, Hesperidin (later down) 3 g, dried ginger and roasted licorice 10 g each. After taking the above 14 doses, the patient was seen again, and the distortion of the mouth and eyes had improved significantly, only slightly shallowing of the left nasolabial fold could be seen when smiling.4 Intercostal neuralgia patient Guo, male, 56 years old, consultation card number: 62333332. May 2, 2013 due to He visited the clinic on May 2, 2013 for “pain in the right hypochondrium for more than 20 days”. He complained of pain in the right side of the hypochondrium, which is obvious when touched, aggravated at night and relieved during the day, no dry mouth or bitterness, sleep is possible, the bowel movements are regulated, the tongue is light and fat, the coating is white and slightly thick, and the pulse is string. Physical examination: no local skin redness, no skin lesions, local pain on palpation, percussion pain in the liver area (-). External ultrasound of liver, gallbladder, pancreas and spleen: slightly enlarged liver, fatty liver, liver cyst, no abnormality of pancreas and spleen. Diagnosis: TCM: hypochondriac pain (evil entering the sympathetic yin); WCM: intercostal neuralgia. I gave hemp with fine spice soup with addition and subtraction. Prescription: Gui Zhi and Roasted Licorice 10 g each, Radix Angelicae Sinensis, Radix Paeoniae Alba, Jujube and Radix Aconiti (decoction first) 15 g each, Hosanna (later) 3 g, Tong Cao 5 g, Ephedra 9 g. 7 doses, 1 dose daily, decoction in water. 1 week later, the patient came to the clinic and reported that the effect began to take effect by the 3rd dose, and the pain disappeared completely by the 5th dose.5 Experience: Ephedra, Hosanna and Aconiti Tang has only three flavors, in which Aconiti warms kidney yang and is the main internal aid, Hosanna The three medicines work together rigorously to drive the evil of Shaoyin out of the body, and to the deficient people who feel cold directly to the disease mechanism. To lead ephedra into the Shao Yin, in order to get out of the Sun into the evil, especially through the Shu Pei combined with the table to warm the meridian, external protection of the Sun’s rigid Qi, internal consolidation of the Shao Yin kidney root, the fluid within the guard, and the Yang does not die outside, this from the inside to the table, from the Yin out of Yang’s agent also.” The first three cases were located on the head and face, and the fourth case was located on the hypochondrium. Modern medicine considers the pathology of these cases to be peripheral nerve axonal degeneration or demyelinating lesions, with the following common features: the onset of the disease is rapid, the disease reaches its peak within a few days, and the onset of the disease is similar to an extrinsic disease, but the patient does not have the manifestations of an extrinsic disease. What kind of evil can cause the destruction of nerve fibers deep inside the organism in a short period of time? The author believes through clinical observation that the rapid onset of the disease in such patients is in accordance with the characteristics of the six externally induced diseases in Chinese medicine. The patient did not have the characteristics of heat, humidity, dryness, fire, etc. The head and face are the high pinnacle, only the wind can reach them, cold is the main attraction, cold is the main stagnation, the patient’s inability to move the eyes flexibly and facial muscle paralysis are caused by cold, the intercostal neuralgia in the fourth case seems to be caused by the cold evil entering the liver meridian of the sympathetic yin, so the wind-cold evil is the main evil causing the disease. Why did this wind-cold evil cause the disease, and why did it enter Shaoyin or Conjunctive Yin without going through the solar phase of the disease? Careful questioning of the patient shows that the patient is mostly a Yang deficiency type of physique, with characteristics of Yang deficiency such as fear of cold, reluctance to drink, and aversion to eating cold, etc. He is not susceptible to external sensations, and once he is ill, he is mostly suffering from seemingly internal injuries such as spleen and stomach. Therefore, it is presumed that to a certain extent, the barrier system of these patients has been damaged, for the reason that the same qi seeks each other, they are susceptible to cold evil, which easily leads to the long drive of external evil to reach the land of Taiyin and Shaoyin, and when the resistance of the body decreases, the external evil enters the land of Shaoyin directly, resulting in the cold evidence of Shaoyin. Therefore, we gave Ephedra Hossein and Radix Pseudostellariae Tang, which hit the disease mechanism and made the disease heal. In the fourth case, because the disease was located in the sympathetic yin meridian, we combined with Danggui Sihui Tang for inducing the meridian.
