Radiation enteritis is a common intestinal complication caused by radiation therapy for pelvic and abdominal malignancies, mainly manifesting as abdominal pain and diarrhea, urgency, anal cramping and blood in stool, etc. It is mostly seen in patients with cervical cancer and colon cancer. The symptoms appear during radiotherapy or within a short time afterwards as acute radiation enteritis, which is generally treated mainly by internal medicine. At present, western medical treatment mainly consists of antibiotics, as well as intestinal mucosa nutrition and protection drugs, but the treatment effect is not good. In recent years, the author used oral Chinese medicine with topical drugs to treat acute radiation enteritis, and achieved better results, the application experience is summarized below. Tumor is a local manifestation of the dysfunction of the internal organs of the whole body, which is caused by the deficiency of disease and the deficiency of reality. Tumor patients have deficiency of vital energy and loss of harmony of Qi and blood, while cervical cancer and colon cancer mostly have deficiency of spleen and kidney as the fundamental pathological mechanism, coupled with radiation irradiation, heat toxicity burns the intestinal organs and further injures Yin and consumes Qi. The spleen and stomach are not healthy, the evil is stagnant in the middle, the qi is stagnant, the intestine is not clear and turbid, the circulation of local qi and blood is obstructed, the qi and blood are not enough to moisten the meridians, and the heat evil hurts the ligaments and moves the blood, thus producing symptoms such as abdominal pain, diarrhea and blood in stool. The symptoms of acute radiation enteritis are mostly mixed with deficiency and actuality, with spleen deficiency as the basis and damp heat as the standard; if the disease is prolonged and becomes chronic, the deficiency of kidney qi is the main cause. The main symptoms of acute radiation enteritis are thin stools, increased frequency, mucus, pus and blood, pain during stool, anal or abdominal swelling, light or red tongue, white fur, and smooth pulse. This disease belongs to the category of “dysentery” or “diarrhea” in traditional Chinese medicine, which is caused by the loss of spleen health, dampness and heat poisoning, and should be treated by strengthening the spleen, removing dampness, clearing heat and detoxification, and harmonizing Qi and blood. Commonly used are Dang Shen, Bai Zhu, Bai Ban Dou, Shan Yao, Gorgonian, Licorice, etc. to strengthen the spleen and benefit the qi; Coix Seed, Poria, Perilla, Chen Pi, and Semen, etc. to permeate the dampness and turbidity; Amaranthus, Fructus Herba, Scutellariae, Bacopa Monniera, and Horsetail, etc. to clear the heat and detoxify the toxin; Paeonia, Mu Xiang, Angelica, Sophora, Citrus Aurantium, and Chuan Xiong, etc. to harmonize the qi and blood. The commonly used formula is Jianhua Qingliang Tang, which consists of raw Astragalus, fried Atractylodes, yam, Sophora, Phellodendron, Pueraria lobata, Fangfeng, raw Job’s tear, Chebulan, Mu Xiang, Red and White peony, Equisetum, Baidong, Jiao Shanzha, and roasted licorice. For obvious abdominal pain, add Yuan Hu and Wu Yao; for mucus in stool, add Che Qian Zi and Dong Gua Ren; for diarrhea and dysentery, add Poppy Shell, Pomegranate Peel and Red Stone Resin; for obvious blood stasis, add Chuan Xiong and Red Vine; for more blood in stool, add Panax Ginseng Powder and Di Yu Char; for kidney deficiency, add Boneset and Puzzle Nut. For more serious cases, we can use Wu Mei Wan plus or minus. The original formula is used to treat ascariasis in the Treatise on Typhoid and Cold, and it is also used to treat prolonged diarrhea. The formula is a combination of acid, sweet and hard work, cold and heat, rigid and soft, the main to start with the sympathetic yin, warming yang and nourishing yin, draining and lowering, taking into account Yangming and Shaoyang, on the basis of which the addition of reduction and tailoring used to treat radiation enteritis, in line with the pathogenesis, and every received good results. The Chinese medicine decoction fumigation or preserved enema can make the drug act directly on the anus or intestinal lesion local, and its treatment is the same as internal tonics, often to dry dampness, detoxify and cool the blood legislation. Commonly used drugs: Atractylodes macrocephala, Phellodendron, Phellodendron, Gynostemma, Fructus, Angelica dahurica, Radix et al. For anal swelling and pain, add Mannitol and Danpi; for more blood in the stool, add Comfrey, Diyu charcoal and Phellodendron leaf; for swelling, add Jiao betel nut and Citrus aurantium shell. 3.Experience Acute radiation enteritis is slow to be treated with Chinese herbal medicine alone, and the relief of anal swelling and pain is not satisfactory. The decoction of Chinese herbal medicine is easy to be accepted by patients because it avoids the stimulation of the gastric mucosa by oral administration of Chinese herbal medicine and is especially suitable for the elderly and weak spleen and stomach, and no obvious toxic side effects are found. The fumigation method is suitable for those with obvious local symptoms in the anus, and can be used alone or together with tonics for internal use to enhance the therapeutic effect. For obvious Qi deficiency, the dosage of Huang Qi can be increased to 60g; for Qi deficiency and subsidence, it is appropriate to reuse Ge Gen and add Sheng Ma; for Spleen and Kidney deficiency and cold, add Si Shen Wan. The large intestine is one of the six internal organs, in order to pass down for the smooth, the dosage of astringent antidiarrheal drugs should not be too large, so as not to retain the evil; the dosage of external treatment drugs should be appropriately increased compared to internal consumption. In addition, the author initially observed that the application of traditional Chinese medicine at the early stage of radiation therapy has a certain preventive effect on acute radiation enteritis, which is consistent with some literature reports [1] and is in line with the treatment idea of “treating the disease before it occurs” in Chinese medicine. Although there are many reports on the prevention and treatment of radiation enteritis with TCM, many of the treatment methods need to be further evaluated, and the specific prevention and treatment effects, the best time of entry and the mechanism of action of TCM still need to be further observed and studied.
