What is “plasmacytoid mastitis”?
It’s also called “ductal dilatation” in the textbooks.
Here I will simply call it “plasma milk”, so that we can communicate better.
There are some characteristics of the initial clinical manifestations of ‘plasma milk’, which you may wish to compare with yourself.
1, often accompanied by congenital sunken nipples
2, often young people, unmarried women are also common
3, often have a history of local trauma to the breast, such as a child’s foot kicked, the child’s head accidentally touched, etc.
4. The symptoms are found in a short period of time
5, often suddenly found breast lumps, along with local pain, skin redness and swelling, some even have other skin rash, low fever and other symptoms
The above are some of the symptoms that accompany the first detection of “pulpy breast”, but as the disease progresses, it does not only stay in the above conditions.
As the disease progresses, it does not stop at the above, but may then appear as follows.
1. The lump gradually becomes redder, more painful, and then softer
2. The softened skin breaks down on its own and pus-like fluid flows out, and the pain is relieved; or the doctor may help to cut and drain it after seeing it.
3.The wound does not heal for a long time, up to several months
4.Or repeatedly red, swollen, painful, broken, repeated wounds do not heal for a long time
Over time, the patient’s mentality changes, leading to a rush to “remove the root”, so much so that the entire breast is cut off, or repeatedly cut and drained, leaving “a thousand holes”, and the breast is seriously disfigured and miserable.
In fact, according to my experience of observing and treating “pulpy breasts” for more than 20 years, pulpy breasts can be divided into several periods: 1.
1. Acute stage: redness, swelling, pain, even low fever and rash
2, chronic sinus period: the skin of the breast breaks down on its own, or the wound does not heal for a long time after the doctor cuts and drains, resulting in the formation of chronic sinus tracts, or even multiple sinus tracts of complementary healing
3, lump type: the initial stage of the disease in the form of lumps, the skin is not red and swollen
4, duct dilatation phase: manifested as congenital nipple depression, thickening of large milk ducts, white or yellow thickened nipples, with slag-like overflow
For the above four periods, the principles of treatment are different for different periods.
1, acute redness and swelling period: adjust the mind, relax, light diet, important conditioning. Not suitable for surgical removal.
2, chronic sinus period: If after the acute period, the wound does not heal for a long time, there is only one sinus tract, and after 3 months of observation, there is no recurrence into the acute period. At this time, you can analyze whether the wound does not heal due to improper medication change? If you can exclude the reason of changing medication, then you can surgically remove the sinus tract and correct the invaginated nipple to achieve a phase of wound healing and reduce the recurrence of pulpy breast.
3, lump type: If the lump is not large, all the lump removal does not affect the appearance of the breast, then you can surgically remove the pathological examination, so as to get a clear diagnosis, but also greatly reduce the possibility of the disease into the acute phase, chronic phase.
4, duct dilatation phase: generally not suitable for surgery, self-care nipples can be, so that the sunken nipples to keep clean for the purpose.
For patients with recurrent acute redness and swelling, self-care is recommended.
1, adjust mood, pay attention to diet
2. drainage by small incisions and avoid long-term placement of drainage strips
3. Look forward to the chance of self-healing of the disease. Avoid impulsive mastectomy and regret for life.
