Dr. Osman from the United Kingdom summarized the current state of research on bladder detrusor hypokinesis: Hypokinesis is an important cause of lower urinary tract symptoms (LUTS) in older patients, but it is significantly under-understood and under-researched. Hypovigilance of the detrusor muscle was proposed by the International Continence Control Society (ICS) in 2002. Urodynamic studies have shown that hyporectalism is present in 9-48% of men with non-neurogenic LUTS and 12-45% of women with LUTS, while the causes of hyporectalism are diverse and include neurogenic (afferent and efferent nerves) as well as myogenic factors. The diagnosis currently relies on urodynamic tests, and the results are not conclusive using only the force of the detrusor muscle as an indicator, as the ability to urinate depends not only on the force of the detrusor muscle, but also on the speed and persistence of the contraction. The current concept is relatively vague and further research is needed to reach a consensus and find the best treatment strategy.
