It is the desire of many young couples to get pregnant and have a healthy baby. There are many reasons for infertility, which are summarized as the problem of “seed” and “soil”. In order to get pregnant, firstly the “seed” must be of good quality and secondly the “soil” must be fertile. In other words, the quality of the egg and sperm should be good, the passage for the sperm-egg encounter should be smooth, and the environment in the uterus should be good; any of the above mentioned abnormalities can cause infertility. There are various causes of infertility and the mechanisms are complex, the main ones being ovulation disorders, blocked fallopian tubes and abnormal uterine function (or organic lesions). There are several cases of ovulation disorders, including follicular dysplasia, which prevents the formation of a dominant follicle, or the inability to expel a dominant follicle after its formation. The fallopian tubes can be affected by developmental malformations, inflammatory infections that form adhesions that prevent fertilization, etc. The uterus can be affected by congenital malformations, dysplasia, endometritis, endometrial adhesions, endometrial insufficiency, etc., which can affect the passage of sperm and the fertilization of eggs. Western medicine mostly uses hormonal ovulation promotion or “IVF” techniques, which are costly and have large individual differences in success rates. The sequelae of ovarian transitional stimulation and reproductive endocrine disorders cannot be ignored. Modern TCM gynecology has developed a set of safe, effective and economical treatment methods by incorporating advanced treatment techniques on the one hand and promoting the advantages of traditional medicine on the other. The actual fact that there is only one original follicle in a normal woman’s menstrual cycle that is mature, also known as the dominant follicle, before ovulation, the follicle can be up to 18-20mm in diameter, and the egg ready to be combined with the sperm is hidden in this large follicle, so the formation of the dominant follicle is a prerequisite for ovulation. Some infertility patients with menstrual disorders are often suffering from follicular dysplasia, which is considered by TCM to be a manifestation of kidney qi deficiency and essence and blood deficiency. On the one hand, we should determine the overall strength and weakness of the patient’s qi and blood, and on the other hand, we should regulate the kidney qi and fill the essence and blood, so that the ovaries can gradually develop their own hormone secretion and eventually promote the development of the dominant follicle. The formation of the dominant follicle is only the first step to success, the small eggs in the follicle can only be released after the follicular membrane ruptures, a process called “ovulation”. If for any reason the eggs are not released smoothly, the umbilical end of the fallopian tube cannot pick up the eggs and the sperm and eggs have no chance to meet and fertilize. This kind of ovulation disorder is also very common in clinical practice and is considered by TCM to be a sign of Yang Qi deficiency. The “Ovulation Promotion Drink”, which warms Yang, moves Qi and invigorates Blood, can help the follicular membrane to rupture and release the egg in time. The “magical meeting” between egg and sperm takes place in a long and thin fallopian tube, without which fertilization cannot be completed. If the fallopian tube is diseased, it can lead to blockage of the official cavity, mainly caused by pelvic inflammatory disease. Generally, you can take Chinese medicine to dissipate the adhesions caused by inflammation, and at the same time unblock the veins so that the fallopian tubes can be reset and unblocked. If combined with Chinese herbal medicine such as retention enema and physiotherapy can enhance the efficacy of the treatment, which usually takes 1 to 3 months. After the sperm and egg meet triumphantly, they eventually come to the uterus for implantation. However, if the environment inside the uterus is poor and the nutrients supplied are not sufficient, it is not conducive to the fertilized egg’s implantation. In some infertile patients, after multiple abortions, the amount of menstruation gradually decreases, accompanied by lumbago and lower abdominal pain, and through hysteroscopy, it is found that the uterine cavity adhesions are strong, decomposition of adhesions to the appropriate treatment. According to Chinese medicine, several abortions and scraping of the uterus damaged the flushing, resulting in deficiency of kidney qi and loss of nourishment and propulsion of the uterus, so that the old blood does not go to stop and stasis remains. Usually 3 months of treatment is needed. Some patients also suffer from uterine insufficiency (including endometrial atrophy, hyperplasia of the endometrium, and luteal phase endometrial insufficiency) leading to infertility, and we have achieved certain efficacy in clinical practice by treating them with Chinese medicine and combining them with modern treatment methods. In conclusion, infertility is a common clinical disease in gynecology with complex causes and many treatment methods, and we believe that individualized treatment plans should be developed. It is recommended that infertility patients use a combination of Chinese and Western medicine to treat both the symptoms and the root cause to improve the success rate of conception. Lastly, we would like to advise young women that many infertility problems can be avoided, and Chinese medicine advocates “treating the disease before it happens”, which means that prevention is the main focus, and women should strengthen their reproductive health care from adolescence. Women with “menstrual disorders”, “endometriosis”, “pelvic inflammatory disease”, “cervicitis”, etc. should be treated as early as possible. The first thing you need to do is to be clean with the opposite sex, master effective contraceptive methods, not to have an abortion as much as possible, and to develop good hygiene habits and lifestyle. This way, when you want to have children, you will be able to be a happy mother.
