Intractable pain is known as the “undead cancer”, how to solve the pain problem is becoming a growing concern in the medical community. The following are a few of the more prevalent neurosurgical treatments at home and abroad in recent years: a. Stimulation 1, spinal cord electrical stimulation (SCS) The main indications are neuropathic pain in one limb, the pain should be relatively limited and fixed in nature. Common disorders include: persistent radicular pain due to failed back surgery syndrome or neuropathic pain associated with complex limited pain syndrome. Neuropathic pain in the trunk area such as postherpetic pain or certain types of post-open-chest pain are also effective for SCS. 2. Intracranial electrical stimulation treatment includes deep brain electrical stimulation (DBS) of the thalamus, paraventricular-parapatellar gray matter (PVG-PAG), and motor cortex electrical stimulation. These methods are mainly used to treat non-cancerous pain, such as failed back surgery syndrome, neuropathic pain due to central or peripheral nervous system injury, or trigeminal neuralgia. the target site of DBS should be selected according to the nature of the pain. Motor cortical stimulation (MCS) is a new treatment method that has gained much attention. It is mainly used for neuropathic pain syndromes and is particularly effective for some intractable facial pain, such as trigeminal neuropathic pain. Since the electrodes are placed in the epidural rather than in the brain, it is safe and effective with few complications. 4.Central drug infusion is a very popular method for the treatment of intractable pain. It has a wide range of indications, including injurious pain and mixed pain, and can be used to treat focal or diffuse pain (such as pain due to systemic bone metastases), as well as axial or limbic pain. Since injurious pain is more sensitive to opioids, the main indications for central drug infusion therapy are pain syndromes with significant injurious pain components such as cancer pain. Destructive therapy Despite the increasing prevalence of stimulant therapy, destructive therapy still occupies an important place in the neurosurgical treatment of pain. In contrast, destructive treatment is more appropriate for injurious pain. Dorsal root of spinal nerve into the medullary zone (DREZ) disruption is a new treatment for intractable trunk or limb pain that has been widely promoted in recent years and is briefly described as follows. It is effective for neuropathic pain in the upper extremities due to cancerous pain such as Pancoast’s syndrome, and is most effective for neuropathic pain due to nerve root avulsions in the neck or lumbosacral region and pain in the limb or “limbic region” due to spinal cord injury. In short, intractable pain is no longer an insurmountable disease, and functional neurosurgeons have been able to defeat this “cancer that will not die”. We hope that patients with intractable pain can receive treatment soon after reading the above introduction, so that they can restore their confidence in life and embrace a better tomorrow.
