Causes of urinary incontinence after electrosurgery of the prostate

It is very common to have transient urinary incontinence after a TURP procedure. The key is to see if there is any damage to the external urethral sphincter during surgery, that is, the scope of electrodes should not cross the seminal frenulum, if the seminal frenulum is also cut that the possibility of damage to the external sphincter is high, then post-operative incontinence may be persistent. The reason for the transient incontinence of TURP is that the internal urethral sphincter has been removed during electrodesection, and the closing function of the urethra can only be compensated by the external urethral sphincter and the surrounding muscular and branching support, and when the strength of these structures is sufficient for bladder pressure, the incontinence stops. So it also takes a few months. So the key is whether there is damage to the external urethral sphincter! 1, mostly pseudo-incontinence, the reasons are: 1, the prostate is too large, compression of the urethral packet sphincter muscle, the latter function is damaged, need time to recover. Can perform anal lifting training, standard anal lifting exercise should be 200 to 300 times per group, three to four groups per day, adhere to one to two weeks. 2, inflammation of the urinary bladder, edema, resulting in urge incontinence, anti-infective treatment and inhibition of bladder spasm treatment. 3, preoperative combined with OAB (overactive bladder disorder), no postoperative bladder training, postoperative diary of urination can be recorded, behavioral exercises to enhance the patient’s self-confidence. 2, true urinary incontinence I am afraid is the external sphincter injury mentioned upstairs, clinical rare, the possible cause is intraoperative injury or postoperative pulling for too long, resulting in impaired sphincter function. Once it happens it is catastrophic and very difficult to treat. 3, if the patient is still no improvement in time, can be reinserted catheterization, line training bladder, record urinary diary, behavior exercise, enhance the patient’s self-confidence.
