How can recurrent wheezing in children be treated and prevented?

  First of all, it is clear that your child basically has a bronchopneumonia with wheezing symptoms induced by a cold, also known clinically as “wheezing bronchopneumonia”, which is frequent especially in the fall and winter seasons. Most of these children have allergies, so the necessary allergen tests can be performed. Long-term prophylaxis is recommended for frequent attacks, either oral or inhalation.  Oral is mainly cisplatin, considering your child’s age is relatively small, inhalation mainly using nebulized inhalation budesonide, or with budesonide aerosol with storage fog tank. Generally follow up promptly after 2-4 weeks of treatment, do not just stop the medication. There is no sufficient basis for your child to be diagnosed with asthma at this time, but there are no specific tests to determine if it is asthma. Therefore, it is currently considered that preventive treatment of asthma is recommended for recurrent wheezing episodes. Preventive treatment for asthma generally requires more than 3 months of medication, and if there are no attacks within 3 months, you can consider downgrading or reducing the dosage of treatment, and if there are no attacks for a year, you can consider stopping the medication. In my personal experience, preventive treatment can be given during the cold-prone season (autumn and winter) for those children who are only triggered after a cold.  The second thing is to reduce the occurrence of colds, which is generally not well done by parents in China, where we mostly think that children catch colds only when they are cold, but in fact the vast majority of children catch colds only when they are hot and then cold. In general, children in families wear more clothes, and there is no problem with wearing more clothes before going to kindergarten, because if children sweat, parents will dry them or reduce their clothes in time, but once they go to kindergarten, you can’t do that. My experience is that in the fall and winter, children can wear at most one more piece of thin clothing than adults (non-elderly), which is not easy to do in China.  Asthma is a manageable disease, and the goal of our treatment is to make these children as normal as possible. About 80% of children’s wheezing will be self-limiting when they grow up, but it is important not to fail to take the necessary preventive treatment because it will be self-limiting, because repeated attacks can lead to a decline in lung function in children and can cause permanent lesions in the airways that cannot be reverted. It is also necessary to correct the view that many parents think that their child wheezes after a cold because of immune deficiency, this view is problematic, this is determined by the child’s specific allergic constitution, indicating that he is particularly sensitive to some things, on the contrary it is a hypersensitivity reaction to these things, not immune deficiency. Exercise can only strengthen the temperament and reduce the number of colds, it does not prevent asthma attacks, on the contrary, some asthma may be triggered after exercise.  It should be understood that proper exercise can strengthen the child’s constitution and reduce the number of colds, thus reducing those wheezing attacks caused by colds. To reduce the attacks of colds, in addition to the usual life attention to dressing, there is also the need to eat a reasonable diet, do not picky food, pay attention to supplement a variety of vitamins, and carry out appropriate physical exercise.
