Gilead II (Harvoni, Harvoni, LEDIFOS) drugs

(Gilead II is a fixed-dose combination of Ledipasvir (a hepatitis C virus (HCV) NS5A inhibitor) and Sofosbuvir (an HCV nucleotide analogue NS5B polymerase inhibitor) for the treatment of chronic hepatitis C in adults with genotypes 1, 4, and 6. (ii) Dosage The recommended dose of LEDIFOS is one tablet taken orally once daily on an empty stomach or with a meal. *The cure rate is up to 98% in patients who have not been previously treated for hepatitis C and up to 94% in patients who have been previously treated for hepatitis C (mainly interferon). Warm tips: Before treatment, hepatitis C viral load (HCV RNA) less than 1 × 106 IU/mL without cirrhosis, primary treatment patients can consider shortening the course of treatment to 8 weeks. (iii) Contraindications None (may be interpreted as a higher drug safety) (iv) Warnings and Precautions Bradycardia due to combination with amiodarone: Severe symptomatic bradycardia may occur in patients taking amiodarone, especially in patients on concomitant beta-blockers or with underlying cardiac complications and/or advanced liver disease. The combination of amiodarone with Gilead II is not recommended. Cardiac monitoring is recommended for use in patients in whom no other viable options are available. (iv) Adverse Reactions The most common adverse reactions (incidence greater than or equal to 10%, all grades) observed after 8, 12, or 24 weeks of Gilead II use were fatigue and headache. (iv) Drug Interactions Coadministration with amiodarone may result in severe symptomatic bradycardia. Concomitant use of Gilead II with amiodarone is not recommended. p-gp inducers (e.g., rifampin, St. John’s wort): may alter concentrations of sofosbuvir and ledipavir. Concomitant use of Gilead II with P-g inducers is not recommended. Please communicate fully with your physician about potential drug interactions prior to administration. (V) Use in Special Populations For the safety and efficacy of medication in pregnancy, lactating women, children, the elderly, people with renal impairment, and people with hepatic impairment, please communicate fully with your physician to understand the safety of medication use.
