In early 2015, a repeat patient came in, accompanied by his lover. Once inside, the patient’s lover complained: Doctor, see if he has ejaculation disorder, he only ejaculates every time he has sex for half an hour, and he is often so tired that he is sweating before he ejaculates, I am tired of being tormented, I have not seen him like this for more than a year since we got married. The patient explained that after treatment, his sexual function has increased and he is able to ejaculate every time, and there is pleasure in ejaculation, unlike the ejaculation disorder described on the Internet. What is going on here? Let’s review this patient’s experience. About 3 months ago, this patient came to our clinic, complaining of poor erectile function for more than a year since he got married, difficulty in vaginal penetration during sex, difficulty in having successful intercourse after numerous attempts, and few morning erections. The patient himself was very distressed and worried about the impact on the relationship between husband and wife and the problem of not being able to have children. After detailed questioning and recording of the patient’s condition, we checked the penile hardness and indicated that there was no erection at all, and then gave a penile color Doppler ultrasound examination indicating vascular impotence (arterial), and combined with the patient’s tongue and pulse, we formulated a treatment plan combining Chinese and Western medicine. After the treatment, the patient’s erection gradually improved and the number of morning erections increased, and he had regular weekly follow-ups and actively cooperated with the treatment. After 3 months of treatment, the scene described at the beginning of the article appeared. I explained the situation in detail to the couple and gave an analogy that, for example, it is inevitable that a person, who used to be poor, now suddenly has a lot of money and feels surprised and uncomfortable. After hearing the explanation, both of them were relieved. Finally, the patient was prescribed another course of medication to consolidate the effect, instructed to exercise more to avoid lack of physical strength during sex, and suggested that preparation for pregnancy could be started. The effective erection of the penis depends on the expansion of the penile artery, the expansion of the blood sinus caused by the relaxation of the smooth muscle of the penile corpus cavernosum, and the closure of the subpubic and conduit veins of the penis. The synergistic effect of these three aspects ensures a full and effective erection of the penis, and the failure of any of these links can cause erectile dysfunction, which is known as vascular impotence. Chinese medicine believes that the cause of arterial impotence may be due to inadequate blood supply to the arteries, which is caused by “stasis” in the blood vessels, and the disease starts with a weak erection and gradually develops into difficulty in erection. The arterial blood supply gradually evolves to “blood stasis”, treatment can be used to tonify the kidney, activate blood circulation and remove blood stasis to improve the blood supply to the penis and restore normal erectile function. The treatment of vascular impotence is a gradual process, and it is necessary to be persistent and insist on the medication.
