Thoracoscopic lobectomy

  Diseases for which thoracoscopic lobectomy is indicated 1. Non-small cell lung cancer (clinical stage I) 2. Benign lung diseases ☆ Bronchiectasis ☆ Pulmonary aspergillosis ☆ Pulmonary isolation ☆ Tuberculosis bulb ☆ Pulmonary cyst ☆ Pulmonary inflammatory pseudotumor Advantages of thoracoscopic surgery 1. Significant reduction in postoperative pain: One of the advantages of thoracoscopic surgery is that it reduces the postoperative pain of patients and reduces the dosage and application time of postoperative analgesic drugs for patients . The postoperative pain in thoracic surgery is mainly related to rib propping, so thoracoscopic lobectomy without rib propping is more in line with the requirements of minimally invasive surgery; the surgical incisions are 2 incisions of about 1.5cm and an incision of about 3.5-4.5cm in length at the anterior axillary line of the 4th intercostal space. (3. Comparison of lung function and mobility: thoracoscopic surgery preserves the integrity of the thorax and the patient’s respiratory function to a large extent compared with conventional open-chest surgery because the chest wall muscles are not cut off and the ribs are not propped open, so the patient’s postoperative lung function and mobility are better than those of conventional open-chest surgery. Some scholars believe that the mediastinal lymph nodes can be completely cleared under thoracoscopy to the extent of open surgery, but others believe that the mediastinal lymph nodes can be cleared less thoroughly and the rate of missed mediastinal lymph nodes is higher in thoracoscopic surgery compared with conventional surgery. Based on our clinical experience, we believe that it is technically safe and feasible to perform thoracoscopic lymph node dissection, which can dissect 2, 3, 4, 7, 8, 9 and 10 groups of lymph nodes on the right side and 5, 6, 7, 8, 9 and 10 groups of lymph nodes on the left side.  The cost is about 10,000 yuan higher than traditional open-heart surgery because only disposable instruments can be used under the lumpectomy to cut large blood vessels and bronchus under the thoracoscope.
